About self-evaluation and external evaluation

For businesses that provide regular patrol / ad-hoc home-visit nursing, small-scale multifunctional home care, dementia-responsive cohabitation care and nursing small-scale multifunctional home care, at least once a year, Based on the standards for personnel, facilities and operation of community-based services, etc., we evaluate the quality of nursing care services provided by ourselves (self-evaluation) and evaluate the results from a third-party perspective at the operation promotion meeting etc. Is required (external evaluation (external evaluation) is required. After the external evaluation, please report the result to the city.

Deregulation of the number of external evaluations

For establishments that meet certain requirements, the number of external evaluations can be eased (once every two years). But, about self-evaluation and creation of target achievement plan, please carry out every year and submit to Yokohama-shi.
Please refer to "Office Administration Guidelines pertaining to the application of relaxation of the number of implementations of Yokohama City External Evaluation" for requirements and procedures to receive the application of the relaxation of the number of times of implementations. In addition, evaluations utilizing the Management Promotion Council cannot be included in the number of years of evaluation, which is a requirement to ease the number of evaluations conducted by outsiders.
