1.Reports at the time of accidents by The Long-term Care Insurance operators

If an accident occurs while providing nursing care services, please take appropriate measures promptly, including contacting the user's family and the home care support office in charge.

2.Procedure for accident reporting using electronic application system

The accident report to Yokohama City is "Yokohama City Electronic Application and Notification Service (hereinafter referred to as" Electronic Application System "). It will be done using it.
After 17:00 on March 25, 2022, the application destination has been changed to the new electronic application system. In order to apply using the new system, "Pre-business registration (login)" is required.

[Caution]
If the screen of the electronic application system is left open for a long time, or if the line is cut off, the entered information may be lost. Please use the "Temporary Save Application" function as appropriate. In addition, items that may become long sentences such as "accident situation, background, response, etc.", "cause of accident", "cause of accident", and "approach to prevent recurrence", should be compiled in a concise sentences in advance. .

  1. After the accident, each company will promptly submit the first report to Yokohama City using the electronic application system.
  2. About progress report of accident processing, we report to Yokohama-shi as needed.
  3. Submit this report (final report) using the electronic application system when there is a break in accident processing.
  4. Each business site actively discloses the contents of the accident report to the user and issues it as requested.

3.Scope of report

  1. Occurrence of user injuries (needs requiring consultation at a medical institution) or fatal accident resulting from the provision of services
  2. Outbreak of food poisoning, infectious diseases and tuberculosis
  3. Occurrence of violations of laws and regulations by employees (employees)
  4. Misstakes (including omissions and medications)
  5. Leaving and missing

4.Obligation of explanation to users, etc.

Actively disclose the contents of the accident report to the user and his / her family, and issue it as requested. In addition, please explain the following contents.

  1. Report the accident to the competent department.
  2. It may be reported to Kanagawa Prefecture as an accident case.
  3. When a request for information disclosure is issued, other than personal information may be disclosed.

5.Others

  1. The responsible section that receives the report will conduct surveys and guidance to businesses as necessary.
