1.Report on accidents at The Long-term Care Insurance operators
If an accident occurs during the provision of nursing care services, take appropriate measures promptly, including contacting the family of the user or the home care support office in charge. .
The Long-term Care Insurance operators are required to prevent recurrence of accidents and respond promptly and appropriately. At the same time, please report to Yokohama City in accordance with the "The Long-term Care Insurance Office Reporting Guidelines for Accidents".

2.Procedures for reporting accidents using the electronic application system
The accident report to Yokohama City is "Yokohama City Electronic Application and Notification Service (hereinafter referred to as" Electronic Application System ") This will be done.
After 17:00 on March 25, 2022, the application destination has been changed to a new electronic application system. When applying using the new system, "advance business registration (login)" is required.

If the screen of the electronic application system is left open for a long time or the line is disconnected, the entered information may be lost. Please use the "Temporary Save Application" function as appropriate. - After the accident, each company immediately submits the first report to Yokohama City using the electronic application system.
- About progress report of accident processing, we report to Yokohama-shi as needed.
- Submit this report (final report) using the electronic application system where there is a break in the accident handling.
- Each business site actively discloses the contents of the accident report to users and issues it upon request.
3.Scope of Report
- Injuries of users (requiring consultation at a medical institution) or fatal accidents due to the provision of services
- Occurrence of food poisoning, infectious disease, tuberculosis
- Occurrence of violations of laws and regulations, scandals, etc. of employees (employees)
- Mistakes (including medication leaks and drops)
- Separation and missing

4.Duty of explanation to users, etc.

- Report the accident to the competent department.
