(5)　Report on fire prevention object inspection results
(6)　Establishment of self-defense firefighting organization (change) registration form
(7)　Fire-fighting equipment, etc. (Special firefighting equipment, etc.) Inspection results report
(8)　Report on the results of disaster prevention management inspections
(9)　Notification of construction of facilities subject to construction maintenance (limited to those with a construction area of approximately less than 1,000 m2)

Attach the "List of attached documents for Perfect Service" and attached documents to the e-mail and forward it to the fire department. (Please do not delete the "reception number" described in the "electronic application completion email" to be forwarded without deleting it.)

In order to be appointed as a fire prevention manager or disaster prevention manager, it is necessary to show a qualification.
