Consultation desk on welfare and health

●In Health and Welfare Center, social workers and public health nurses provide consultation on welfare and health. The contents of the consultation include the elderly (including applications for The Long-term Care Insurance), physical, intellectual, mental disorders, single-parent homes, DV, etc.
After consultation, we take over to each person in charge and provide appropriate services.
●Familiar Community Care Plaza (Local Elderly Care Management Center) also provides consultation on elderly people.
Consultation time (common to each facility) Month to Saturday: ９: 00-18: 00/holidays: ９: 00～17:00
※Closed days vary depending on the facility, so please contact each facility.
※Closed days and closed hours are connected to the call center for consultation, but we do not accept day service (elderly day care) holidays or contact care managers. Please contact us at the consultation time of the Community Care Plaza.

●If you have any questions, please contact us. We will guide you about the welfare system, the independence support system for the poor in life and other available systems.
●Under the independence support system for the poor in life, we provide consultation on finding jobs and rebuilding households in cooperation with Hello Work.
