●In Health and Welfare Center, social workers and public health nurses provide consultation on welfare and health. Consultations include elderly people (including applications for The Long-term Care Insurance, etc.), physical disabilities, intellectual disabilities, mental disorders, single-parent families, and DV.
After consultation, we take over to each person in charge to provide appropriate services.
●The familiar community care plaza (Local Elderly Care Management Center) also provides consultation on the elderly.
Consultation time (common to each facility) Month-Sat: ９: 00-18:00 Days/holidays: ９: 00～17:00
※Closed days vary depending on the facility, so please contact each facility.
※On closed days and closed hours, you will be connected to a consultation call center, but we do not accept day service (elderly day care) holidays or contact care managers. Please contact us at the consultation time of the Community Care Plaza.
