Birth Report FormThis is a postcard attached to the Maternal and Child Health Handbook. Please submit it as soon as possible after birth. The birth contact form will be used for visiting business.

Child Allowance

Those who are raising elementary school student until the end of junior high school (the first March 31 at the age of 15) will be paid from the month following the date of the request.

Raising Premature Infants medical care benefitsIf a baby weighing less than 2,000g or a baby (0-year-old child) born with immature physical development is hospitalized at a designated medical institution, in principle, we will support the co-payment of insurance medical treatment and meals at the time of hospitalization .
