・Yokohama City local recyclables collection slip and application form for grantResources and Waste Recycling Bureau
　　Cut the top and bottom along the cut line.
　　The upper half (for submission to Resources and Waste Recycling Bureau)
　　Please write down the points in the confirmation column and mail it.
　　The lower half is a copy of the registered organization.
