・Yokohama City local recyclables collection slip and incentive grant application form (for submission to Resources and Waste Recycling Bureau)
　　Cut up and down along the cut line.
　　For the first half (for submission by Resources and Waste Recycling Bureau)
　　Please write the letter in the confirmation column and mail it.
　　The lower half will be a copy of the registered organization.
