About National Health Insurance limit application authorization

There is no need to present a limit application certificate. ※You do not need to apply for renewal every August.
If you use a minor insurance card, you will be exempted from paying more than the limit in the high medical expenses system without prior procedures. You do not need to apply in advance for the limit application certificate, so please use your minor insurance card.
With the introduction of the online qualification confirmation system at medical institutions, etc., the limit application category can be confirmed with the consent of the individual, so the limit application certificate (for tax-exempt households, "limit application / standard burden reduction certificate ") is not required.
※Paper limit application certificates, etc. can be used as before.
※Those who fall under the following need to apply for a limit application certificate.
・If you are going to a medical institution that does not have an online qualification confirmation system
・When households exempt from municipal tax who have been hospitalized for more than 90 days in 12 months prior to the application date receive further reductions in dietary expenses
If you wish to receive a paper limit application certificate, etc.
Please apply at the ward office Insurance and Pension Division in your ward office.

- Identity verification documents (My number card, driver's license, passport, etc.) of the person who came to the notification
※The effective date of the limit certificate is from the first day of the application month. If the amount paid in the previous month exceeds the co-payment limit, you must apply for high medical costs.
