Based on the results of specific medical examinations, the level of the need for lifestyle improvement is determined based on the number and age of risk factors (hypertension, lipid abnormalities, hyperglycemia, smoking habits) in lifestyle-related diseases.
By this judgment, those who fall under "motivation support" or "active support" are subject to specific health guidance.

Specific health guidance is a three-month support program for those who fall under "motivation support" or "active support" to set lifestyle goals and implement them. Specialists, such as doctors, public health nurses, and dietitians, can help you to carry out your daily life without difficulty.

Contents of motivation support
Motivation support is for those who have begun to risk metabolic syndrome.
(1)First Interview
We look back on the results of specific medical examinations and your own life (lifestyle), receive advice from doctors, public health nurses, dietitians, etc., and set action goals that can be achieved in three months.
(2)Evaluation after 3 months
Check changes in physical condition and lifestyle.
Contents of Active Support
Active support is for those with overlapping risks of metabolic syndrome.
(1)First Interview
We look back on the results of specific medical examinations and your own life (lifestyle), receive advice from doctors, public health nurses, dietitians, etc., and set action goals that can be achieved in three months.
(2)Continuous support
We will continue to receive support for practices necessary for improving lifestyle habits, such as nutrition and exercise.
(3)Evaluation after 3 months
Check changes in physical condition and lifestyle.
It is important not only to try health guidance, but also to look back and confirm your hard work and physical changes for three months.
Free (There is no co-payment for specific health guidance.)
Individuals who are subject to specific health guidance (those who are determined to be eligible for specific health guidance as a result of specific medical examinations) will be mailed individually to "specific health guidance use tickets" to use specific health guidance. .

1 Confirmation of the contents of the use ticket
Among the items sent to the user, the use ticket is a Uguisu-color paper.
Please confirm the target person's name, expiration date, and specific health guidance category ("motivation support" or "active support") listed on the use ticket.
2 About reservation for use of specific health guidance
If you wish to use specific health guidance, please select a designated health guidance company and make a reservation and contact the desired company directly.

Precautions for Reservation
- Specific health guidance can be used only by those whose name is listed on the voucher.
- Person who stopped Yokohama-shi National Health Insurance cannot use specific health guidance even if we have use ticket.
- When making a reservation by phone, please bring your ticket at hand. In addition, please tell that you want to use specific health guidance of Yokohama-shi National Health Insurance.
- Please note that the expiration date indicated on the voucher is the deadline until the initial guidance of specific health guidance is used. Please make a reservation as soon as possible so that you can receive the first instruction within the expiration date.
