About procedure of medical system for elder senior citizens by mail

For the next procedure of the medical system for elder senior citizens, notification and application by mail are possible. Please note that we may contact you or return if there are any deficiencies.

Issuance of Qualification Confirmation, etc.

If you lose your certificate of insured medical care for the elderly (insurance card) (reissuance of the insurance card has been completed) 

Issuance of medical insurance card for the elderly (insurance card) for the elderly ended on December 1, 2024. If you lose your medical insurance card (insurance card) for the elderly after December 2, 2024, you will be issued a new qualification confirmation. Please fill in the required items on the application form for the issuance of the Medical Certificate for Elderly Senior Citizens and the optional entries, and send it to Insurance and Pension Division National Health Insurance Section, the ward office of your ward. I will send you a confirmation of eligibility. A power of attorney is required if someone other than the person or a relative of same household applies in proxy/agent.

About application of "qualification confirmation" pertaining to person requiring consideration

If a third party such as an assistant needs to accompany the person to assist in confirming the qualification, even if you have a minor insurance card, you will need to apply for a minor insurance card. By doing so, you can receive a "qualification confirmation".
Please fill in the required items on the application form for the issuance of the Medical Qualification Confirmation Form for Elderly Senior Citizens and the optional items, and send a copy of your identity verification document to Insurance and Pension Division National Health Insurance Section, the ward office in your ward. A power of attorney is required if someone other than the person or a relative of same household applies in proxy/agent.

About limit application certificate and limit application, standard burden reduction certificate (reduction certificate)

Grant of limit application certificate and limit application, standard burden reduction certificate (reduction certificate) were finished on December 1, 2024.

The valid limit application certificate and the limit application / standard burden reduction certificate (reduction certificate) that you have as of December 1, 2024 can be used until the expiration date after December 2. (Up to July 31, 2025).
However, if there are any changes to the information, such as Address or applicable categories, it will not be usable.
In the future, those who do not have a valid limit application certificate and a limit application / standard burden reduction certificate (reduction certificate) will use a minor insurance card or indicate the classification of the limit. By presenting the “Qualification Confirmation Form” (*), you will be exempted from paying exceeding the limit.

※ The classification of the limit is described by application, but those who have received the limit application certificate and the limit application / standard burden reduction certificate (reduction certificate) after August 1, 2024 Will be issued a qualification confirmation stating the category of the limit without applying.

About cancellation of use registration of minor insurance card

Please fill out the application form for cancellation of registration of your My Number Card Health Insurance Card and send a copy of your identity verification documents to Insurance and Pension Division National Health Insurance Sectionrequired items, the ward office in your ward. If someone other than the person applies in proxy/agent, a power of attorney is required.

Benefits

When applying by mail, it is necessary to print out the application form distributed at the counter (see table below) by yourself.
In the case of application by mail, we may contact you to confirm the contents. In addition, it may take more time to transfer money than usual.

※A copy of the passport (passport) must be accompanied by the applicant's name and a copy of the immigration record pertaining to the application.

※If you are unable to confirm the mourner and the funeral day, please contact the ward office Insurance and Pension Division National Health Insurance Section. It may be necessary to attach a petition for payment of funeral expenses.
