About procedure when we moved out of Yokohama city
You do not need to apply for transfer to the ward office Elderly and Disabled Support Division in Yokohama City.
If you are transferred out of Yokohama City, you will not be able to use the designated medical expenses (designated incurable disease) recipient ID card in Yokohama City because the information will change.
For this reason, please apply for a specific medical expenses in charge of the designated incurable disease department (if the local government is a government-designated city, the government-designated city is responsible for the transfer procedure of the resident certificate, and the prefectural government is responsible for other municipalities).
Depending on the local government, the window of the public health center (it may not be possible to apply at the municipal office where you move in) and required documents are different, so please contact the department in charge directly.

About transfer procedure from outside Yokohama city
If you are transferred to Yokohama City and use the specified medical expenses (designated incurable disease) subsidy program as before moving in, you will need to apply for a new application to Yokohama City.
