Yokohama City Medical Institution Online Support Project Subsidy (Designated incurable disease)

Overview

In both designated incurable disease and Chronic Pediatric Chronic Diseases, a new incurable disease-Kokoko database (“incurable disease Kokoko DB”) was established. From October 1, 2023, a medical opinion statement and a clinical survey personal card can be registered directly from medical institutions on April 1, 2024.
Until now, patients have submitted clinical survey personal slips prepared by designated physicians to local governments, and local governments have registered the clinical survey personal slips in incurable disease and Koboku DB by sending copies of medical opinion papers to the Pharmaceutical Foundation, Health and Nutrition Research Institute, and medical opinion papers to the National Center for Growth and Medical Care.
By enabling online registration of clinical survey personal votes and medical opinion statements (hereinafter referred to as clinical survey personal votes, etc.), the time required for registration will be shortened, patient data can be used promptly, and treatment research will be further advanced. It is expected that this will lead to the benefit of patients, such as the development of treatment methods such as the development of new drugs and the formulation of medical guidelines. In addition, a number of functions have been added to improve the efficiency of office work.
In order to register online, it is necessary to establish a system environment to meet various requirements, such as terminal requirements and connection requirements. In order to promote the improvement of the environment for this online registration, we have established the "Yokohama City Medical Institution Online Support Project Subsidy". Therefore, please consider the use of this subsidy.

Supplementary contents

Target institutions: Medical institutions that will respond online in FY2024
Subsidized amount: Up to 50,000 yen (base amount: 1/2)
Example 1) Required amount 50,000 yen
   50,000 x 1/2 = 25,000 yen
Example 2) Required amount 35,000 yen
   35,000 x 1/2 = 17,500 yen ⇒ Since less than 1,000 yen will be rounded down, 17,000 yen will be issued.
Example 3) Required amount 150,000 yen
   Since the maximum grant is 50,000 yen, the grant amount is 50,000 yen.
Subsidized: Purchase expenses for PC terminals and printers
     Expenses for improving network environment
     System renovation costs within medical institutions
Date of delivery: middle of March, 2025

Grant application (deadline: until 17:00 on Friday, November 15, 2024) ※Fiscal 2024 ended.

Please submit one of the following. In addition, attached documents is requested to submit a document that shows the items you plan to purchase and the amount of the items. (Please note that subsidies cannot be issued if purchased documents, such as delivery notes and invoices, are attached.)

 ・Specifications for PC terminals and printers (pamphlets or catalogs are also acceptable)
 ・Materials on network environment improvement and system renovation of medical institutions
 ・Other reference materials

Notes

・Only the required amount for online support will be eligible for application. Purchases and renovations before receiving the grant decision notice from Yokohama City, if you have already completed online support, or applications for the second and subsequent times are not eligible for the subsidy.

・Please note that subsidies may not be issued if the budget is exceeded.

About issuance of designated doctor ID and PW

An ID and PW are required for each designated physician to create a clinical survey personal card within incurable disease / Koko DB. ID and PW issuance will be accepted as follows, so if you are a medical institution that can cooperate with the online registration of the medical certificate, please apply. For more information, please refer to the following link.
If you are a medical institution that has already applied for ID / password issuance, please wait for a while until the ID / password issuance notice and the media containing the programs required for online registration are sent.

Notes

・Please apply at the medical institution level.
・If more than one designated doctor belongs to a medical institution, please apply for more than one person.
・The designated physician listed on the first line will be granted the "responsible" authority for the first system configuration.
・The only designated physician to apply is the designated physician whose main employer is the medical institution of the application.
・Application is limited to medical institutions in Yokohama city.
・At the time of application, please indicate the name of the medical institution and the date of application in the file name. (Example: [XXX Hospital] [20230915] Medical Institution User Data File.xlsx ")
・Please do not change the form of the application form and submit it as it is. (Please do not convert it to CSV format.)
・Application acceptance on this page will be accepted as a designated doctor for incurable disease and a designated doctor for cooperating incurable disease.
 

Flow after application

1 Yokohama City registers the information of the designated doctor you have applied for in incurable disease Koko DB.
2 After registration, a medium containing information for the designated doctor to connect to the national DB will be sent to Yokohama City from incurable disease Koko DB operator.
3 Yokohama City encloses the ID and PW issuance letter of advice and sends the media to the applicant medical institution.
4. Please refer to the explanatory materials in the medium and prepare for confirmation of connection to incurable disease/Kokodo DB.
