[Important] Notice of renewal of identification of recipient of specific medical expenses (designated incurable disease) in FY2024

About renewal procedure of identification of specific medical expenses (designated incurable disease) recipient ID in 2024

The validity period of your current recipient ID will end on September 30, 2024.
Renewal procedures are required to receive the recipient ID from October 1, 2024.
For required documents, etc., please refer to the information on the procedure for renewing the identification of the recipient of the specified medical expenses (designated incurable disease) in FY2024 (PDF: 1,511KB).

[Handling Content]
If the renewal application procedure is completed by the following application date and the certification is obtained, a recipient ID will be issued from October 1, 2024.
 ・Date of application 
 In the case of mail application Monday, September 30, 2024 ※Postmark valid on the day
 In the case of application at each ward Elderly and Disabled Support Division window Monday, September 30, 2024

 ※You can apply for renewal until the above date, but you will receive a new recipient ID by October 1, 2024.
  The application deadline for application is [must arrive on July 31, 2024 (Wednesday).]

Information on renewal procedures

If you already have a recipient ID, we will send out a set of guidance documents for renewal procedures sequentially from early June 2024.
If you do not receive it by mid-June, please contact the Health and Social Welfare Bureau Medical Aid Division, incurable disease Countermeasure Section.

To alleviate congestion at the ward office, we recommend mailing application. In the case of mail application, please attach a stamp for the required amount of money.

(It is recommended to mail by a method that allows you to check the delivery status such as simple registered mail.)


Precautions

・If there is a change in the contents of the current recipient ID, a change application is required. For details, please refer to the Specific medical expenses (designated incurable disease) Subsidy Program (change application) page.

・In the case of mail application, the application acceptance date is the date when the documents are opened and accepted by the incurable disease Countermeasures Section of the Medical Assistance Division.
