[Important] Notice regarding renewal of specific medical expenses (designated incurable disease) recipient ID for FY2024

About procedure of renewing recipient ID of specific medical expenses (designated incurable disease) in FY2024

The validity period of your current recipient ID will end on September 30, 2024.
Renewal procedure is necessary to receive identification of recipient from October 1, 2024.
For required documents, etc., please refer to the information on procedures for renewing recipients of the specified medical expenses (designated incurable disease) in FY2024 (PDF: 1,511KB).

[Handling Details]
If you apply for renewal by the following application date and are certified, you will be issued a recipient ID from October 1, 2024.
 ・Date of application 
 In the case of mail application Monday, September 30, 2024 ※Postmark valid on the day
 In case of application at each ward Elderly and Disabled Support Division window Monday, September 30, 2024

 ※You can apply for renewal until the above date, but you will receive a new recipient ID by October 1, 2024.
  The application deadline for this is [must arrive on Wednesday, July 31, 2024].

About sending out information about renewal procedure

For those who already have a beneficiary ID, a set of renewal procedures will be sent out sequentially from early June 2024.
If you do not receive it by mid-June, please contact the Health and Social Welfare Bureau Medical Aid Division incurable disease Countermeasure Section.

Notes

・If there is a change in the contents of the current recipient ID, an application for change is required. Details can be found on the specific medical expenses (designated incurable disease) Grant Program (application for change) page.

・In the case of mail application, the application acceptance date will be the date when the documents were opened and accepted by the Medical Aid Division incurable disease Countermeasures Section.

・Designated physicians and designated medical institutions in Yokohama are listed on the specific medical expenses (designated incurable disease) subsidy program (list of designated doctors and designated medical institutions).

Notice of recipient ID after renewal application

About the end date of the validity period of the recipient ID from October 1, 2024

 In order to alleviate window congestion, the end date of the validity period has been changed as follows from the recipient ID after the renewal application in 2024. As a result, the application period for renewal of the recipient ID will also be changed.
 The end date of the validity period of the beneficiary ID from October 1, 2024 depends on the patient's own birth month. Please check the validity period chart below. (* If you are apportioned within same household, the expiration date may differ from the validity period chart.)

The next renewal time is as described in the validity period chart. It is also printed at the bottom of the address of the recipient ID from October 1, 2024 sent this time, so please check it.
