List of designated physicians designated by Yokohama City

In order to be certified by the medical expenses under the Special medical expenses (Designated incurable disease) Grant Program
It is necessary to submit a clinical survey personal card described by a designated doctor designated by the prefectural governor or a government-designated city mayor.
There are two types of designated doctors.
・incurable disease designated physician…Doctors who can fill out the clinical survey personal form required for new and renewal applications
・Cooperation incurable disease designated physician…Doctors who can fill out the clinical survey personal card required for renewal application

When requesting a doctor to fill out a clinical survey personal card, make sure that the doctor is the designated doctor before requesting.
※Please note that designated doctors apply daily, so depending on the timing of updating the homepage, it may not be posted yet.

List of designated medical institutions designated by Yokohama City

A designated medical institution is when a medical institution receives medical treatment related to the designated incurable disease.
Specific medical expenses (designated incurable disease) This refers to a medical institution that can be subsidized by medical expenses.
Please note that medical treatment received at a medical institution that is not a designated medical institution is not eligible for refund (if there is no special provision in laws and regulations).

If you already have a beneficiary ID, you can receive a medical examination at a medical institution designated by a prefecture or government-designated city, including Yokohama city.
(When it is necessary to have an emergency medical examination on the go, etc., even medical institutions that are not listed on the recipient ID can be used if they are specified.)
