In order to receive certification from the medical expenses under the specified medical expenses (designated incurable disease) subsidy program.
It is necessary to submit a clinical survey personal card written by a designated doctor designated by the prefectural governor or the mayor of a government ordinance-designated city mayor.
There are two types of designated doctors.
・Dr. incurable disease Designated Doctor…Doctors who can fill out the clinical survey personal form required for new applications and renewal applications
・Dr. incurable disease…Doctor who can fill out the clinical survey personal form required for renewal application

When requesting a doctor to fill out a clinical survey personal card, make sure that the doctor is a designated doctor before requesting.
※Please note that designated physicians are required every day, so depending on the timing of updating the homepage, it may not be posted yet.

A designated medical institution is when receiving medical treatment pertaining to the designated incurable disease at that medical institution
Medical institutions that can receive subsidies from medical expenses under the specified medical expenses (designated incurable disease) subsidy program.
Please note that medical treatment received at a medical institution that is not a designated medical institution will not be eligible for refund (unless otherwise specified by law).

If you already have a recipient ID, you can receive a medical examination at a medical institution designated by prefectures or government ordinance-designated cities nationwide, including Yokohama city.
(If you need to consult urgently on the go, etc., even a medical institution that is not listed on the recipient's ID can be used as long as it is specified.)
