Application for re-issuance of Specified medical expenses (designated incurable disease) recipient ID

If you have lost, soiled or damaged your beneficiary ID, fill out the application form for re-grant of the recipient ID and apply.
When applying for a reissue of a recipient ID after the death of the recipient of a specified medical expenses.
Attachment of "Application for Application and Receipt of Yokohama City Specific medical expenses" is required. (It is not necessary to attach certificate of family register etc.)

If you return your beneficiary ID due to healing or death, please fill out and apply for the recipient ID Return registration form.
※Please attach the recipient ID when returning the card. In addition, when returning, liquidation of designated medical institution (visit nursing station, home-visit nursing care provider, etc.) is required.
