Yokohama Municipal Hospital Medical Charter
We will promote the following five items so that citizens who use the hospital can receive high-quality medical services safely and safely.
- We will respect the voices of patients and provide medical services based on mutual trust.
- We will respect the right of patients to know.
- We will accurately provide informed consent (explanation and agreement based on patient understanding and selection) and respect the patient's right to self-determination.
- We will respect the privacy of patients.

Patient's Rights
- You can receive high-quality medical care equally.
- Human rights as individuals are respected.
- Personal information and privacy are protected.
- You can know your medical information.
- Symptoms, diagnosis, treatment, and future prospects can be explained in easy-to-understand words.
- With sufficient explanation, you can choose or reject tests and treatments on your own.
- You can hear the opinions of other doctors about diagnosis and treatment.
Responsibilities of Patients
- You have a duty to adhere to hospital rules and take care not to interfere with the medical care of other patients.
- In order to ensure the safety of medical care and enhance therapeutic effects, we have a duty to cooperate with medical care, such as providing accurate information about your health.
- We have a duty to receive explanations about the cost of medical treatment and to pay medical expenses appropriately.
