Professionals provide consultation at the counter or visit, such as guidance on medical institutions, various information, and nursing care.
If you have any doubts, please consult here first.

Regional Comprehensive Support Center (Community Care Plaza)

　We carry out health and welfare consultation about dementia by specialist doctors by visiting the ward office or visit. (About once a month in each ward)
　Please refer to each ward office (the above) for schedule and conduct contents.

It looks different from before, or maybe dementia? If you think so, please consult your GP first.
※If you do not have a family doctor, first consult a general consultation desk (above).

・List of medical institutions that diagnose and treat dementia
We publish information of medical institution performing medical treatment of dementia in the city.

・Yokohama City Medical Center for Dementia Disease
It is one of the specialized medical institutions for dementia. Consultation is possible by telephone or interview (reservation required).

・Dementia support physicians and family doctor
We release participant list of dementia support doctor training training and family doctor dementia responsiveness improvement training.

We can consult with you about care methods and concerns about nursing care.

It is possible for caregivers to interact and exchange information. Gathering of caregivers is carried out at Local Elderly Care Management Centers.
