Summary of Yokohama City Dementia Initial Intensive Support Project (Dementia Intensive Support Team)
Purpose
In order to respect the will of people with dementia even if they have dementia and to continue living in a good environment in the area where they are accustomed to living as much as possible, the `` Dementia Initial Intensive Support Team '' for people with dementia and their families at an early stage. And build a support system for early diagnosis and early response of dementia.
Details of Business
A team composed of professionals visits the homes of people with dementia who are not connected to medical care or nursing care, observes and evaluates, consult medical institutions and support the use of nursing care services, We provide comprehensive and intensive support such as advice according to the state of dementia (approximately 6 months) and provide support for independence support.
※Team composition: It consists of two or more people who have national qualifications for medical health and welfare, such as public health nurses, nurses, occupational therapists, mental health workers, and care workers, and one or more specialists with dementia.
Targets
Persons living at home who are suspected of having dementia over the age of 40 or who have dementia who fall under any of 1 or 2
- Person who does not receive medical or nursing care service
- People who receive medical and nursing care services, but have severe symptoms due to dementia and are having difficulty responding

- Coordination of consultation with specialized medical institutions
- Support for use of nursing care services
- Psychological support and advice to caregivers in accordance with the state of dementia
- Care for the body, such as diet and exercise, and improvement of living environment
