Overview of Yokohama City Dementia Initial Intensive Support Project (Initial Dementia Intensive Support Team)
Purpose
In order to respect the will of people with dementia even if they have dementia and continue to live in a good environment in the area where they live as much as possible, the `` Dementia Initial Intensive Support Team '' for early diagnosis and early response of dementia. We will establish a support system for early diagnosis and early response of dementia.
Contents of Business
A team composed of professionals visits the homes of people with dementia who are not connected to medical care or nursing care or suspected dementia, observes and evaluates them, supports consultation at medical institutions and use of nursing care services, We provide comprehensive and intensive support such as advice according to the condition of dementia (approximately 6 months) and support independence support.
※Team configuration: It consists of two or more persons with national qualifications for medical health and welfare, such as public health nurses, nurses, occupational therapists, mental health workers, and care workers, and at least one specialist with dementia.
Targets
People who are living at home and are suspected of having dementia or who are over 40 years old and have dementia who fall under any of 1 or 2
- Persons who have not received medical or nursing care services or who are interrupted
- People who receive medical and nursing care services, but have strong symptoms due to dementia and have difficulty responding
Contents of Support
- Coordinating consultation with specialized medical institutions
- Support for the use of nursing care services
- Psychological support and advice to caregivers according to the state of dementia
- Care to improve the body, such as eating and exercising, and improvement of living environment
