About Long-term Care Insurance Premium's fund transfer

1 Application using the WEB fund transfer service

　In addition to the fund transfer request form, there is a web account transfer service that does not require seal to fill out troublesome documents anytime, anywhere, 24 hours a day.
　(Because the financial institutions that can use this service are limited, please check the website.)

2 Mailing to financial institutions and ward offices and applications by the next agency
　Please fill out the bank account transfer request form with required items, submit the passbook registration stamp to the financial institution or ward office with deposit and savings after seal.
　The fund transfer request form is available at the financial institution and ward office Insurance and Pension Division National Health Insurance Section.
　In addition to the fund transfer request form, the customer's cash card can be read into a dedicated terminal installed at the ward office window.
　There is also a "Pay-Bank Transfer Reception Service", which electronically registers fund transfer with a financial institution via a dedicated network.

3 Notes
　Please refer to ward office Insurance and Pension Division of your ward about change or cancellation of transfer account of fund transfer.
　If you are 65 years old or have been transferred to Yokohama City, you will be required to pay by normal collection for a while.
　After that, either special collection (deduction from pension) or ordinary collection (payment by payment slip or fund transfer)
　The payment method is stipulated by laws and regulations, and cannot be selected.
