Refund of Long-term Care Insurance Premium

 As a result of recalculation of Long-term Care Insurance Premium due to transfer, death, etc., an overpaid amount (error payment) may occur when insurance premiums fall or when insurance premiums for the same delivery date are duplicated. In the event of an incorrect payment, it will be returned to the insured or heir as a refund.
 Eligible persons will be notified in the “Refund Notice for Long-term Care Insurance Premium, etc.” In addition, the “Refund Notice for Long-term Care Insurance Premium, etc.” contains the documents required for the procedure for receiving the refund (request for refund account transfer), so please fill out the required items and return it in the enclosed reply envelope. (If you have already registered for direct debit for Long-term Care Insurance Premium payment, you do not need to complete the procedure.)
 Please fill in the refund account transfer request form referring to the entry example below.
※Please be careful about fraudulent phone calls that pretend to be Yokohama City staff. We will not ask you to operate the ATM for refunds.
