Consultation Desk

If you are considering using disability welfare services, please feel free to consult with Health and Welfare Center (Ward Office) Elderly and Disabled Support Division or Children and Families Support Division in your ward. The consultation desk varies depending on the age of the person who uses the service.

※Window on the opening day of the ward office Reception hours is from 8:45 am to 5:00 pm Monday to Friday (excluding holidays and holidays, from December 29 to January 3). Disability welfare services include a variety of other services besides legal services. The ward's Health and Welfare Center (ward office) offers a wide range of consultations, including legal services, as well as other services. In addition, if necessary, we will guide you on the creation of a “service usage plan”.

We will guide you through the available services according to the content of your consultation. The required application procedures and contact points may differ depending on the service provided. Please feel free to contact us.

The flow of the procedure

From application to payment decision

If the use of the service requires the certification of the disability support category (nursing care benefits, etc.), it will take approximately two months for the service to be available after receiving the payment decision from the application. It may take more time depending on individual circumstances.

Oh "Application"

You can apply at the window of the ward office. If the procedure at the counter is difficult, please contact the consultation counter in your ward by telephone.
※The application may not be the ward office of your ward, such as a special place of residence. If you have any questions, please contact the office of your ward.

Oh “Certification Survey”
We visit applicant's home from ward office and carry out interview investigation. After applying, we will inform you of the schedule of your visit. Interviews will be conducted on certification survey items (80 items) and the status of everyday life.
Please refer to the “Disability Support Category” (external site) of the Ministry of Health, Labour and Welfare for an overview of the accreditation survey.

Oh "Doctor's opinion" (when applying for the use of services (care benefits, etc.) that require certification of "disability support categories")
We ask medical institution having you appoint at the time of application to make "doctor opinion statement" from ward office. In many cases, the medical institution submits a “doctor's opinion” in 2-3 weeks, but depending on the usual situation of outpatient visits, the medical institution will ask you to consult again to create a “doctor's opinion”. May be asked. In addition, depending on individual circumstances, it may take some time before the "doctor's opinion" is submitted by the medical institution.

[Request for designated medical institutions]
Please specify a medical institution that fully understands the condition of the applicant's disability. In some cases, it may not be possible to create a doctor's opinion because the content of the disability is not specialized, or the content of the written opinion regarding the disability status may be insufficient. It is recommended that you check with a medical institution in advance to see if it is possible to enter the “Doctor's Opinion for Disability Support Category Certification”. In addition, Yokohama City prepares a pre-examination vote (PDF: 641KB) for preparing a doctor's opinion. Please use it by submitting it to a medical institution if necessary.

Oh "Examination / judgment by the Disability Support Category Certification Examination Committee" (when applying for the use of services (care benefits, etc.) that require certification of "Disability Support Category")
The results of the "doctor's opinion" and the "certification survey" are computerized (primary judgment), and based on the results and the contents heard at the time of the survey, the secondary judgment is made at the "disability support category certification examination committee", and the classification and certification validity period is determined. Classification is "Not applicable", "Category 1" to "Category 6", and the certification validity period is certified between 6 months and 36 months.

Oh Issuance of “Payment Decision” and “Recipient ID”
Payment of service is decided in ward office in reference to authorized obstacle support division and "use plan such as services", and identification of recipient is issued.

Creation of a service usage plan

Do you know "Consultation Support Office"?

Health and Welfare Center of each ward will make a service payment decision with reference to the "Service Usage Plan" prepared by designated specific consultation support offices.
The consultation support specialist at the consultation support office confirms the necessary services for the person and formulates a service usage plan. In addition, we will coordinate the use of services with service providers. Even after starting using the service, in cooperation with the service provider, regular monitoring and meeting staff will be held to review the type and amount of service.
This is information such as the acceptance situation of designated specific consultation support establishment (information for citizens [plan consultation support])

The applicant (including his / her family and supporter) can also create a service usage plan (self-plan).
※As a general rule, The Long-term Care Insurance's services take precedence for those aged 65 or older who are covered by the long-term care insurance system (40 years or older in the case of No. 2 insured persons with specific illness). If you wish to use both The Long-term Care Insurance Service and Disability Welfare Service, we will make a payment decision referring to the care plan prepared by the care manager instead of the service usage plan. ※We do not accept applications by e-mail from a security perspective.
Oh It is necessary to fill out two types of applications: "Application for Payment of Disability Welfare Services" and "Application for Reduction and Exemption of User Burden".
Oh When you fill out the application form, please receive an explanation of the system or an explanation of the handling of personal information from the ward office staff before filling in. If the application is incomplete or missing, the application may not be accepted, or it may take some time to confirm. If you have any questions, please contact us.
Oh Please fill in contact Phone number etc. that can be contacted during the day (8: 45-17: 00) on weekdays (Monday to Friday) from the person in charge of the ward. If the application is incomplete, it may not be possible to confirm it, and the application may be returned. Thank you for your understanding.

If multiple service establishments are used within the same month, and the service usage fee may exceed the user's upper limit monthly limit, based on this notification, the upper limit management establishment determined in advance by the ward will manage the upper limit. Perform management.
