Subsidy for transportation expenses to attend facilities (physical, intellectual, mental)

Transportation expenses (*) for pick-up and drop-off assistants such as the person and family, required to go to Psychiatry Day Care, etc. We support through facilities (elementary school student facilities) (we are targeted for tsusho outside the city).
※Excludes the amount borne by other systems.

1 Target person

Persons with disabilities who are 15 years old or older who live in Yokohama City and go to the hospital to receive the following business, mainly public transportation (trains / buses) or private cars (4) , * Limited to cases where there is no means of commuting other than a private car due to the situation of disability, etc.)

2 Billing/Payment Date

Bills and payments will be made twice a year for the first half (April 1-September 30) and the second half (October 1-March 31).

(1) About the subsidy unit price per tsusho road

The subsidy unit price per tsusho refers to the main place of residence (the place where the number of days of living occupies the majority and is the base of living) to the tsusho facility (if you receive pick-up and drop-off by facility vehicle from the middle) Is the amount per one-way determined in advance based on the main tsusho method (tsusho method and tsusho route).
Even if multiple tsusho methods are assumed, the unit price is determined based on one of the main tsusho methods. When claiming transportation expenses, even if there is a day when you actually go to another route, you will be paid based on the determined subsidy unit price.
(If it becomes normal to use another tsusho method, a change application is required.)

In the case of public transportation (trains / buses), the subsidy unit price is the amount equivalent to the fare reflecting discounts for persons with disabilities, etc., and 1.5 times the normal fare for assistants (accompanying) The number of rides that have been performed is eligible for the grant). In addition, the amount equivalent to the 6-month commuter pass fee is determined as the upper limit.
In the case of a private car (four-wheeled vehicle), it is 20 yen (rounded up to less than 1 km) per 1 km of the shortest route from the main place of residence to tsusho facilities.

However, the subsidy unit price is economical (amount) and rationality (speed, convenience, etc.). From the viewpoint of the disability situation), the city will confirm the appropriate tsusho route. In addition, in the case of a route (such as making a detour) that is very different from the path that is normally considered due to a failure.

(2) Expenses not covered by the grant

The following items are not eligible for the grant.

  1. Fares for transportation (buses operating in the city, municipal subway, Kanazawa Seaside Line) that can use tickets, etc., for those who meet the requirements for obtaining a special welfare ticket or a special elderly ticket. (Not applicable regardless of whether or not a ticket is actually obtained.)
    Requirements for obtaining special welfare tickets: Handbook for the physically handicapped 1st to 4th grades, love notebook A1 to B2 (including judgment documents), Mental Handicapped Handbook 1st to 3rd grade
    Requirements to obtain a special ride certificate for the elderly: 70 years old or older
  2. Transportation expenses for pick-up and drop-off assistants when a resident goes to the office with the help of a guide helper of the "Disability Support Project"
  3. Transportation expenses during the trial tsusho period
  4. Transportation expenses when going to a facility that provides employment transition support to receive an assessment (employment assessment) for the use of the type B payment decision service
  5. Transportation expenses for social security recipients to go to Psychiatry Day Care, Psychiatry Short Care, Psychiatry Night Care or Psychiatry Day Night Care.

Figures not eligible for grant

Unit price determination applications and billing procedures are performed by tsusho facilities, etc., which have been entrusted with clerical work from the resident.
In that case, we will use the "tsusho expenses subsidy system".
※We do not accept applications or requests from individual residents.

1 Facility Registration
(1) Submission of facility registration sheet

This is a pre-registration so that only facilities that perform billing work can use the tsusho transportation expenses system.
This registration is required only once for each facility.
To ensure security, the number of PCs that can use the system is limited to two.
At the time of facility registration, it is necessary to register the MAC address of the computer that uses the system.
Please fill in the following file and send it by e-mail.

・Subject of E-mail: Please specify "Registration of the facility for subsidizing transportation expenses".

In addition, when there is a change in the registered contents, it is necessary to change the facility information as appropriate. Change work can be done on the system for "TEL number", "FAX number", "email address", "facility manager name", and "account information". <System Usage Manual>

Other items cannot be done on the system, so please fill out the facility information change sheet and send it by e-mail.

(2) Obtain a login ID and password (mailed from the city)

Based on the contents of the facility registration application, after registering the facility in Yokohama City, the login ID and password for using the system will be mailed from Yokohama City to each facility.

About user who started tsusho, we grasp tsusho route information from user and have user submit power of attorney (seal) from user.
※If your main place of residence is different from Address in resident certificate, you must attach a document (such as a utility bill receipt) that confirms the actual residence status to the city when applying for the unit price determination and submit it to the city.

※At the time of application for unit price, it is not necessary to send a letter of attorney to Yokohama City (a copy is mailed at the time of request).

(2) Application for unit price determination (using the tsusho expenses subsidy system) <System Usage Manual>

Based on the tsusho route information, apply for the subsidy unit price by the system by the 20th of the month following the start of the tsusho.

  1. Based on the tsusho route information, apply for the subsidy unit price by the system by the 20th of the month following the month of the tsusho start month (postal application to Yokohama City is not required).
    ※For those who have started tsusho in March and September, the application deadline is April 10 and October 10, respectively. (Because it is necessary to enter the claim by April 20 and October 20.) ※Even if the unit price application and billing are not in time, you can apply for and claim the unit price retroactively for a certain period of time.
  2. Print out the application information from the tsusho transportation cost system and check with the user to see if there is no error in the application details (personal information and tsusho route). If there is an error in the application details, correct the application on the system and reapply.
    ※The subsidy unit price is displayed in the printed application information, but please note that if there is an error in the application content, the unit price after examination and decision in Yokohama City may change the amount from the time of application.

(3) Determination of subsidy unit price (displayed in the subsidy system) <System Usage Manual>

Yokohama City will review the unit price application information received by the 20th of each month at the end of the month, and the determined unit price will be displayed on the system at the beginning of the following month.
If there is an error in the application content, the unit price at the time of application and the determined unit price may differ, so please check with the system to see if there is any person whose application unit price and the determined unit price.
※If the user cannot be identified with the information submitted (the name, date of birth, Address, etc.), the application information may be returned.

(4) Print the determined unit price and confirm it with the user.

For users whose application unit price and the determined unit price are different, print out the system screen and inform the user of the determined unit price.
※Users with the same application unit price and the determined unit price are checked with the user at the time of application, so there is no need to reconfirm after the decision.

(5) Change in subsidy unit price <System Usage Manual>

If the disability situation or the main tsusho status changes, you will need to apply for a “unit price decision” (change application).

(Example)

  1. When the person's notebook status changes (when a new notebook is acquired, when the grade of the notebook changes, or when the notebook is abolished, etc.)
  2. When the main place of residence of the resident changes (for example, when moving)
  3. When the main route changes (mainly when the transportation used changes, etc.)
  4. When the main attendance status of the pick-up assistant has changed (e.g., when the pick-up assistant needs to be accompanied (unnecessary))

(1) Input billing information input and invoice printing (system) <System Usage Manual>

Enter the billing information (such as the number of visits for each user) in the tsusho transportation system and print the invoice. For each user and the total bill, the tsusho transportation expenses system automatically calculates.
The input is for the first half (for April 1 to September 30), October 1 to October 20, and the second half (for October 1-March 31), April. Please go from 1 to April 20.

(2) Submission of invoice and power of attorney (mail)

Please seal the representative's seal on the printed invoice and mail it to Yokohama City with a power of attorney (copy). (Submit of attorney (copy) is submitted once a year (at the time of the first request to go to the office))
Mailing is valid for postmarks, and the first half (for April 1 to September 30) from October 1 to October 20, and for the second half (October 1 to March 31) Please go from April 1 to April 20. (* If the deadline for submission is a closed day, the next open agency date will be the submission deadline (postmark valid))

※We will review the invoice in Yokohama City and contact the facility if there is a leak in seal. In that case, the facility should resubmit the revised bill to Yokohama City as necessary.

(3) Payment of transportation expenses

  1. Transfer the amount stated on the invoice from Yokohama City to the account of each facility. In addition, it is possible to check the scheduled date of transfer to each account using the tsusho expenses system. (You will be able to confirm in mid-November for the first half and mid-May for the second half.)
  2. Please hand the transportation expenses transferred from Yokohama City to each user and receive seal in the payment ledger, and prepare documents certifying receipt (a copy of bank transfer etc.) (preserved for 5 years)

Up to the final submission deadline in the table below, in addition to the normal billing documents, a request can be made by submitting a “Reason for Delay” stating the reason for the delay in submission in addition to the normal billing documents.
