Anyone who intends to participate in this proposal shall meet the following requirements:
(1)Being listed in Yokohama City General Competitive Bidding Qualified Person List (related to articles, consignment, etc.). Or, at the time of submitting the participation intention application form, the applicant has applied for registration in the Yokohama City General Competitive Bidding Qualified Person List (related to Goods, consignment, etc.), and registration is expected to be completed by the date to identify the contract candidate That is.
(2)Person who has not received nomination suspension according to rule of measures summary such as Yokohama-shi nomination suspension from deadline of presentation of participation intention application form to specific day of contract candidate.
(3)Persons who do not fall under the provisions of Article 167-4 of the Local Autonomy Law Enforcement Ordinance (1947 Cabinet Order No. 16).
(4)Have obtained the Privacy Mark or ISO 27001/ISMS.
(5)In data analysis, a person who can analyze medical data such as Yokohama-scale receipts can be assigned.
(6)Being able to assign doctors, nurses, public health nurses, pharmacists, and other equivalent medical professionals in responding to inquiries regarding recommendation notices, etc.
(7)Having a track record of similar projects in other local governments.

　After submitting the application form for participation (Form 1) and written oath (Guideline 1), check the eligibility for proposal in Yokohama City, and regardless of qualification, participate by Friday, April 9, 2021. We will send you a qualification confirmation result notice (Form 2) by mail and e-mail.
　At the same time, we will send a proposal-related document submission request form (Form 6) by mail or e-mail.
