The conventional style (including the attachment) has been abolished due to the change in the style of the important matter description (introduction of the information disclosure system import style).
In the future, please use the new style listed in Table 3.
In addition, the new important matter manual (information disclosure system import style) is a format created by the government and cannot be edited as follows.
(Example) ・ Change the font size and font size
   ・Changes in the height of rows and width of columns in Excel
   ・Editing cells other than cells that can be input/edited (inserting or deleting arbitrary contents, etc.)

About accident report of 4 house-type paid nursing home
When accident occurs in house type paid nursing home, please make report, and I would like report to Yokohama-shi.
(1) Scope of Accident Report
(A) Injuries or fatal accidents of users due to the provision of services
B. Food poisoning, infectious disease, outbreak of tuberculosis
C) Violations of laws and regulations, scandals, etc. of employees (employees)

(2) Procedures for Reporting Accidents
(A) Contact the family of the user, the home care support office in charge, etc.

(C) This report shall be made at a time when the accident processing is separated (electronic application) ※Store copies at the facility
D) Each business site actively discloses accident reports to users and issues them upon request.

(3) Duty of explanation to users, etc.

A. Create an accident report and submit it to the responsible section.

B. Cases of accidents may be reported to the national government, Kanagawa Prefecture, etc.

C) When an information disclosure request is issued, other than personal information may be disclosed.


(4) Notes
A. Make sure that there is no blank space in the entry field of the accident report (if there is no, fill in "No")
Please indicate in the “Contact with Related Organizations” that you have contacted the family of the user or the home care support office in charge of the City of Yokohama, Health and Social Welfare Bureau Elderly Facilities Division and the user's family.

Oh, when an accident report is submitted, the facts may be confirmed with the user or his / her family.
