1. Tuberculosis designated medical institution

◆What is a tuberculosis designated medical institution?
 Tuberculosis designated medical institution is an institution in charge of public medical care for tuberculosis patients prescribed in Article 38, Paragraph 2 of the Act on Prevention of Infectious Diseases and Medical Care for Patients with Infectious Diseases. If you receive the designation, you will be obliged to comply with the "Infectious Diseases Designated Medical Institution Medical Care Regulations (PDF: 159KB)".
 Tuberculosis designated medical institutions include hospitals, clinics, and pharmacies.
 Unless it is a tuberculosis designated medical institution, in principle, you will not be able to provide tuberculosis publicly funded medical care.

1.About designation

About tuberculosis designation medical institution in Yokohama-shi, director of Yokohama Ichibo appoints based on application of establisher.

2.Responsibilities of tuberculosis designated medical institutions

 Tuberculosis designated medical institutions must be in charge of medical care at public expense, as required by the Act on Prevention of Infectious Diseases and Medical Care for Patients with Infectious Diseases.

3.Application method (the format has been changed since April 2022)

(1) When applying for designation of a new tuberculosis designated medical institution
 Applicant: Establishment of hospital, clinics or pharmacy
 Documents to be submitted: ・Tuberculosis designation medical institution designation application (PDF: 102KB)
      ・Documents confirming that the medical institution is a medical institution (opening permit, copy of registration form, etc.)

(2) If you decline a tuberculosis designated medical institution
 Applicant: The establisher of a tuberculosis designated medical institution (or his / her family if the establisher dies, etc.)
 Documents to be submitted: ・Notification of decline in tuberculosis designated medical institution (PDF: 66KB)
      ・Tuberculosis Designated Medical Institution Designated Form (Original)

(3) When there is a change in the specified contents
 If there is a change in the designated medical institution, the procedure will differ depending on the content.

 Oh, when we decline current designation, and new designation application is necessary
  (a) When the establisher changes
  (b) When changing the establisher from an individual to a corporation or from a corporation to an individual
  (c) When moving a medical institution (including temporary relocation due to extension or renovation)
  (e) When changing clinics to hospital and clinics

 (A) When it is necessary to submit a change notification
  (a) When the name of a medical institution is simply changed without a change in the contents of the medical institution
  (b) When there is a change in the name and lot number of the location name of the medical institution due to a change in the display of the house.
  (c) When the name of the establisher is changed due to marriage, adoption, corporation name change, etc.
