In the procedures related to pharmaceutical affairs, the presentation and submission of the original qualification certificate has been changed to the presentation and submission of a copy.
In the procedures related to pharmaceutical affairs, the presentation of the original qualification certificate (pharmacists' license, sales engage registration certificate, course completion certificate, etc.) at the counter has been changed to a copy presentation (submission). In addition, the submission of the original qualification certificate (diploma, etc.) has been changed to a copy submission. Be sure to check the original at the responsibility of the business operator that has obtained (acquires) the permission. It is not necessary to state in the copy to be presented / submitted to indicate that the original certificate has been certified.
Documents other than the qualification certificate (certificate of family register (extract) book, entry certificate, medical certificate, etc.) remain unchanged.

Regarding the notification of the number of prescriptions handled, we will start accepting electronic application from January 1, 2024!
About notification of the number of handling prescriptions, we will be able to report 24 hours a day, 365 days a year using the Yokohama City Electronic Application and Notification System. All pharmacies in Yokohama City are eligible. For details, please see the guide (PDF: 998KB) for users.
In addition, we do not issue a “copy (duplicate)” of the notification by electronic application. If you need a public health center reception stamp on your copy, please use the paper-based procedure as before.
The law on ensuring the quality, efficacy and safety of pharmaceuticals and medical devices has been revised.
The conventional "officers who perform business" has been abolished, and the name of "executive responsible for business related to pharmaceutical affairs" will be included in the application form.
Please state the name of an officer responsible for the business related to pharmaceutical affairs as of August 1, 2021 and the disqualification clause of such officer in the notification of change submitted after August 1, 2021.
However, this is not required if you have already submitted and submitted it in another notification of change or permission renewal.
In addition, the procedures under the Poisonous and Deleterious Substances Control Law have not changed from before.
