※The procedures for business operators of poisonous and deleterious substances, users of specific poisons, and researchers of specific poisons are handled by the Medical Care Bureau Medical Safety Division (045-671-3876).

In the procedures related to pharmaceutical affairs, the presentation and submission of the original certificate of qualification has been changed to presentation and submission of a copy.
In the procedures related to pharmaceutical affairs, the presentation of the original certificate of qualification (pharmacist's license, sales engagement certificate, course completion certificate, etc.) at the counter has been changed to presentation (submission) of a copy. In addition, the original certificate of qualification (diploma, etc.) has been changed to a copy. Be sure to check the original at the responsibility of the business operator who has obtained (acquired) permission. It is not necessary to state that the original has been certified on the copy presented and submitted.
Documents other than the certificate of qualification (certificate of family register (extract) book, registration certificate, medical certificate, etc.) remain the same as before.

Regarding the notification of the number of prescriptions handled, we have started accepting electronic applications on January 1, 2024!
About report of the number of handling prescriptions, we became able to report using Yokohama-shi electronic application, report system. All pharmacies in Yokohama are eligible. For details, please see the guide for users (PDF: 922KB).
In addition, we do not issue a “copy (duplicate)” of notification at the reception by electronic application. If you need a seal from the public health center, please use the paper-based procedure as before.
The law on ensuring the quality, efficacy and safety of pharmaceuticals and medical devices has been revised.
The conventional "officer performing business" has been abolished, and the name of "officer responsible for business related to pharmaceutical affairs" has been stated in the application form.
In the notification of change submitted on or after August 1, 2021, please state the name of the officer responsible for the business related to pharmaceutical affairs as of August 1, 2021 and the eligibility of the officer's disqualification clause.
However, this is not required if you have already submitted it in another notification of change or permission renewal application.
In addition, procedures under the Poisonous and Deleterious Substances Control Law remain unchanged.
