Vaccinations is expected to increase resistance (immunity) by inoculating vaccines that have been confirmed to be effective in preventing disease and preventing disease from becoming more severe.

For those who do not have a pre-examination card due to transfer or loss
For those who do not have a pre-examination vote due to transference or loss, after confirming the past vaccination history at the window of the ward, we will issue the necessary pre-examination vote.
①Documents confirming the name, date of birth, and Address of the person to be vaccinated
②Mother and Child Handbook (such as original documents that show past vaccination history)
Please bring it to the ward office Health and Welfare Division Health Promotion Section (12th floor on the second floor).
When we receive routine immunization of child at a medical institution outside the city
Advance procedures are required at the ward office Health Promotion Section.
Based on the Vaccination Law, we will issue a “Vaccination Implementation Request Form”, which is a document that clarifies that The Mayor of Yokohama is responsible for its implementation.
As of the day of vaccination, there is a resident registration in Tsurumi Ward, and routine immunization at a medical institution outside the city for reasons such as returning home outside the city (including the birth of a younger brother and sister) or inpatient treatment at a medical institution outside the city. If you would like to receive vaccinations, please complete the procedure.
Vaccination costs outside the city are paid entirely by yourself, but in the case of the above reasons, you can receive a refund (reimbursement payment) of the vaccination fee by receiving a “Vaccination Implementation Request Form” in advance. Information on refund method will be mailed together with the “Vaccination Implementation Request Form”.
