We are working to help you apply for a My Number Card!

We set up application support meeting place of my number card in commercial facilities in the city and carry out assistance of application such as photography of face photograph.

What can be done at the application support venue

You can complete the procedure for free.

I will help you fill out the application form.

We will help you fill out the application form for your My Number Card.

If you have an application (*) for My Number Card, please bring that application.
※We send application of my number card by the following method.

  • Enclosed with My Number Notification Card or Personal Number Notification
  • For those under 75 years of age who have not yet applied, mail at the following time:
    • From February to March 2013
    • From July to September 2004
    • From early November to early December of Reiwa 4
  • Mailed to those who are 75 years old or older and have not yet applied from mid-February to late Reiwa 4

If you do not have an application form, we will distribute a blank application form at the application support venue, but please check in advance as your My Number (12 digits) is required. After receiving the application form and envelope at the application support venue, you can fill it out at home.
If you do not find an application at home and do not know your My Number, please come to the ward office.

I'll take a photo of my face.

Take a free photo of your face to be displayed on your My Number Card. Print immediately on the spot and attach it to the application form.
※At the special center, you may take a photo of your face using a tablet terminal and apply online.

We will give you an envelope for sending the application form.

We hand envelope to send application to local public entity information system mechanism (my number card issuance center of country).
The postage is free, so please enclose the application form, seal it, and post it to the post.

The Sakae Ward Office also provides application support.

※The window is crowded on Saturday when the agency is open. Please come with plenty of time.
※Sakae Ward Office is available only to residents of Sakae Ward.

Notes

  1. Those who live in the city can use it. (The Sakae Ward Office is available only to those who live in Sakae Ward.)
  2. Those who apply for a My Number Card for the first time can use it.
  3. You will not be able to receive your My Number Card at the application support booth. Please pick it up at the My Number Card Special Center or Ward Office.

Receiving

About one month after applying, you will receive a grant letter (postcard) from the ward office.
Please receive it at the My Number Card Special Center (reservation required) or Sakae Ward Office (reservation required) according to the guidance in the delivery notice.
