We are helping to apply for a My Number Card!

We set up application support venue of my number card in commercial facilities in the city and carry out help with applications such as photography of face photograph.

What can be done at the application support venue

You can apply for it for free.

I will help you fill out the application form.

I will help you fill out the My Number Card application form.

If you have a My Number Card application (*), please bring the application form.
※We send application of my number card by the following method.

  • Enclosed with My Number Notification Card or Personal Number Notice
  • For those under 75 years of age and have not yet applied, mail it at the following time:
    • From February to March, Reiwa 3
    • From July to September, Reiwa 4
    • Early November to the beginning of December, Reiwa 4
  • For those who are 75 years old or older and have not yet applied, mail will be sent from mid-February 4.

If you do not have an application form, we will distribute a blank application form at the application support venue, but you will need to enter your My Number (12 digits), so please check in advance. After receiving the application form and envelope at the application support venue, you can fill it out at home.
If you do not find the application form at home and do not know your My Number, please come to the ward office.

I'll take a picture of your face.

Take a photo of the face displayed on my number card free of charge. Print out on the spot and attach it to the application form.
※At the special center, you may use a tablet terminal to take a photo of your face and apply online.

I will give you an envelope for sending the application form.

We hand envelope to send application to local government information system organization (my number card issuance center of country).
The shipping cost is free, so please enclose, seal it, and post it to the post.

※The window is crowded on Saturday open agency day. Please come with plenty of time.
※The Sakae Ward Office is available only to residents of Sakae Ward.

Precautions

  1. Those who live in the city can use it. (The Sakae Ward Office is available only to residents of Sakae Ward.)
  2. It is available to those who apply for a My Number Card for the first time.
  3. You will not be able to receive your My Number Card at the application support booth. Please pick it up at my number card special center or ward office.

About receipt

In about one month after applying, you will receive a grant letter (postcard) from the ward office.
Please receive it at the My Number Card Special Center (reservation required) or the Sakae Ward Office (reservation required) according to the guidance on the delivery notice.
