Target animals: From March 1, 2024 to February 28, 2025, "cats without owners" that live in the city that had surgery at a registered animal hospital.

（５） Notice Points Payment will be made to the applicant's account approximately 3 to 4 months after applying.

1 Selection of veterinary hospitals

　From "cat sterilization castration promotion business registration animal hospital list" (PDF: 150KB), please choose animal hospital to carry out operation.
　Be sure to make an appointment for surgery by phone in advance, and tell them that you will apply for a subsidy from Yokohama City at that time.

2 Application Documents

（１） You can download it from the window of Health and Welfare Center Health Sanitation Division in each ward, below.

（２） Perform sterilization and ear cutting at a registered animal hospital, and after surgery, receive the “Sterile castration operation certificate” and “receipt” described in the animal hospital.

　Please fill in the cat's individual No. on the "Sterile Castration Implementation Certificate" and prepare one copy of the receipt.

　In addition, the “Sterile Castration Surgery Implementation Certificate” must be filled out with the applicant's entry in advance to the registered animal hospital at the time of sterilization castration operation.

　Please submit it.

（３） Take a color photo that allows you to check the characteristics and ear cuts of the target cat after surgery (the camera must be prepared by the applicant).
　If it is difficult to take photos after surgery, please consult a veterinary clinic.

3 Application method　
　Please prepare the application documents and apply at the Health Sanitation Division Health and Welfare Center or Yokohama Animal Protection Center.
　Please keep a copy of your application in a safe place until the grant is transferred.

　It is also possible for proxy/agent to apply (a power of attorney is required).
　Application by mail, fax, or e-mail is not possible.

4 Grant (non-grant) decision of subsidies　

　We mail subsidy grant decision letter of advice (or subsidy non-grant decision letter) to applicant from Yokohama-shi animal protection center.

　(Approximately 2-3 months after applying)

　In about one month after that, a subsidy will be paid to the applicant's account (it takes about 3 to 4 months from application to payment).
　※During the year-end and New Year holidays and when applications are concentrated, it may take some time to pay.

3 application forms (1 original, 2 copies)
　※seal is not required for the application form.
　※seal is required to delegate the application. If you want to correct a mistake, a correction mark is also required.

2 Breakdown of Application Amount (1 copy)

　Required when applying for more than two animals.

3 Power of Attorney

　Required when applying for proxy/agent.
　You need seal with the same vermilion mark as the application form.

4 Surgery Implementation Certificate (original) issued by a registered animal hospital

　Follow the steps below to create a document.
　（１） Obtain the form of the certificate of operation and fill in the applicant's Address, name, Phone number, hair color, pattern, and location of the target cat.
　（２） Submit to a veterinary clinic at the time of surgery.
　（３） After surgery, you will be asked to issue it as a certificate of the animal hospital.
　（４） Enter the cat's individual number so that all cats applying for at the same time have a serial number.
※The cat's capture location must be filled in up to the lot number.

5 Receipt issued by the registered animal hospital (original and one copy)

　Be sure to confirm that the address is the full name of the applicant and there is no error when receiving the receipt.

6 Color photo of the target cat

7 Identity Confirmation Documents (original)

　Issued by a public institution with the current address.
　In the case of an application for proxy/agent, the applicant's identity verification document (original or copy) and proxy/agent's identity verification document (original) are required.
