　Please make a reservation by phone in advance and tell us that you will apply for a subsidy from Yokohama City at that time.

2 Application Documents
（１） Microchip attachment subsidy grant application

（２） Copy of receipt of attached treatment of registered animal hospital
（３） Registration certificate issued by the designated registration organization and documents that can confirm the date of microchip installation

　　　・Receipt issued by registered hospital (only if there is a description of the date of wearing microchip)

3 Application method
　Once you have the application documents, please bring it to the Animal Protection Center or apply by mail.
