This page provides some application forms. About style which we do not offer, please use thing which we distribute in presentation (ward office) of application.
Applications and notifications cannot be made via the Internet such as e-mail.
Please submit the declaration form to the ward office.

Application and notification forms are provided in PDF (PortableDocumentFormat). To view and print PDF files, AdobeReader, which is distributed free of charge by Adobe Systems Incorporated, is required. If you do not have it, download and install AdobeReader (external site) from the website of Adobe Systems, Inc. before using.
Please use plain paper for printing and print.
Please use A4 size white plain paper for the declaration form. Please note that if the size and printing surface are different, it may not be accepted. Please do not use thermal paper, backing paper, colored paper, etc.
Please note the following points when printing the application / notification form.
When printing the declaration form, display the form on the screen and press the print button on AcrobatReader. You cannot print the application / notification form with the print button on the browser. For inquiries about entry methods and procedures, please contact the application submission address (ward office).
