Application forms, guide (things related to bath tax)

1 Not all application and notification forms are provided.
This page provides some application forms. About style which we do not offer, please use thing which we distribute at presentation of application (Finance Bureau corporation taxation section bathing tax charge).

2 Please submit the tax return by mail or bring it to the submission destination in principle (Finance Bureau Corporate Taxation Division, Bath Tax Section).
※You can also submit the "Support Bath Management Declaration Form" by e-mail.

3 "Adobe Reader" is required. I need it.
Application and notification styles are provided in PDF (Portable Document Format). To view and print PDF files, Adobe Reader, which is distributed free of charge by Adobe Systems Incorporated, is required. If you do not have it, download and install Adobe Reader (external site) from the Adobe Systems Incorporated site before using it.

4 Use plain paper to print and print.
Please use A4 size white plain paper for the declaration form. Please note that if the size and printing surface are different, it may not be accepted. Please do not use thermal paper, backing paper, colored paper, etc.

5. Please note the following points when printing the application / notification form.
When printing the declaration form, display the form on the screen and press the print button on Acrobat Reader. You cannot print the application / notification form with the print button on the browser.

The tax return form may be changed (corrected) due to revisions to the ordinances and rules, so please check the latest information when you reserve a report form.
