Payment of funeral expenses

When a member of the National Health Insurance dies, a funeral fee is paid to the person who performed the funeral.

Payment amount: 50,000 yen

<Applicant>
Those who performed the funeral when a member of the National Health Insurance dies

<Required for Application>

• Identification documents of the applicant
• Documents that can confirm the mourner (such as a receipt of a funeral shop, an invoice or a funeral note)
• A copy of bank deposits and savings passbooks or account numbers of financial institutions

※If you are transferring to an account other than the applicant, you will need private seal (the applicant's name uses vermilion).

<Application>
The ward office Insurance and Pension Division of the ward where the deceased lived

※Please note that the application destination is not the Insurance and Pension Division of the ward where you live, but the Insurance and Pension Division of the ward where the deceased lived.
※If you have it, please bring the insurance card of the deceased at the time of application.
※Please note that two years after the funeral, the statute of limitations will expire and you will not be able to apply.
※In cases 1, 2, and 3 below, even if you have taken out National Health Insurance at the time of death, you will be paid a burial fee from the former employee health insurance. For detailed procedures, please contact the former employee health insurance.

• If paid from the previously subscribed employee health insurance

1. If you had previously joined employee health insurance within 3 months prior to death as an insured person.
2. In the event of death or within 3 months prior to death, if you have received continuous benefits from the previously enrolled employee health insurance
3. In the event of death or within three months prior to death, the employee received continuous benefits from the previously enrolled employee health insurance.
