About authorization, confirmation change of authorized child institution

If you are going to change the contents of certification based on the Act on Promotion of Comprehensive Provision of Education and Childcare for Preschool Children and the contents of confirmation based on the Child and Child Care Support Law, you need to apply or report. Therefore, please complete the procedures appropriately by the specified deadline.
※We may ask you to correct the contents of your submission.

Changes to facilities

(1) Name of authorized child institution, (2) change of location of authorized child institution with house indication ※ Notification style (name of authorized child institution, facility location) (word: 27KB)

 ※When the display of the location is changed without relocation due to the indication of the house, etc.

 ※When relocating and renovating, it corresponds to (3).
