About change of authorization, confirmation of local type childcare business

When we are going to change contents of authorization based on the Child Welfare Law and confirmation based on the Child and Child Care Support Law, it is necessary to apply or report. Therefore, please complete the procedures appropriately by the specified deadline.
※If there is no prior consultation, you may not be able to change the content or you may be asked to correct the content.

Documents to be submitted and required documents

The deadline and required documents differ depending on the content of the application for change and notification of change pertaining to approval and confirmation.
Please check the submission deadline and required documents before preparing.

Change application / change notification form (Amendment on April 1, 2022)

The forms required for the procedures for changing approval and confirmation items are as follows.
Please be sure to use this form for the procedure.
※If the notification form is divided into two types, word and excel, please create and submit both.
(Applications are required separately for setting approval, abolition and suspension of community-based childcare business.)

Changes to facilities

(1) Name, (3) Change of Location due to indication of residence※ 
  Notification style (name of facility, facility location) (word: 38KB)

 ※When the display of the location is changed without relocation due to the indication of the house, etc.

 ※When relocating and renovating, it corresponds to (4).

(2) Type

(4) Facility size (area of each room, use category, site and outdoor playground area, etc.)

 ※To change the capacity, please consult with the Children and Families Support Division of the facility where the facility is located in advance and contact this section.

Changes related to corporations

(1) Name of the installer (corporation, etc.) and location of the main office

(2) Representative (the person in charge of management (name, date of birth, Address))
  Report style (representative) (word: 41KB)

(3) Articles of Incorporation, Donation Act, etc. and Certificate of Registration 
  Notification Form (Articles of Incorporation, etc.) (word: 37KB)

(4) Officers (name, date of birth, Address)
  Notification style 1 (executive) (word: 39KB) Notification style 2 (officer) (Excel: 38KB)

Other changes ※The submission destination is different.

(1) Matters related to claims for payment of benefits, etc. (Change in the name of transfer account)
