About authorization, confirmation change of local childcare business

When we are going to change contents of authorization based on Child Welfare Law and confirmation based on Child, child care support law, it is necessary to apply or report. Therefore, please perform the procedure appropriately by the specified deadline.
※If there is no prior consultation, the content may not be changed or we may ask you to correct the content.

Documents to be submitted and required documents

The submission deadline and required documents may differ depending on the content of the application and notification of change related to approval and confirmation.
Please check the deadline and required documents in advance before preparing.

※The deadline for submitting a notification of approval / confirmation change concerning the name of the regional childcare business is one month before the date of change.
 However, the submission deadline for reflecting the name of the new garden in the online application form (for parents) is as follows.
 If you want to reflect the new facility name on the desired garden selection screen of the online application [for simultaneous entrance in April of the following year], please submit it by the end of July.
 In addition, if you want to reflect the new facility name on the desired garden selection screen of the online application [May of the following year to March of the following year], please submit it by the end of December.

About change application, change notification style (summary revision on April 1, 2022)

The format required for the procedure for changing approval and confirmation items is as follows.
Please be sure to use this form for the procedure.
※If the notification style is divided into two types, word and Excel, please submit both after making.
(Applications are required separately for the establishment approval, abolition and suspension of local childcare business.)

Changes related to facilities

(1) Name, (3) Change of location due to indication of residence, etc.※

Changes related to corporations

(1) Name of the installer (corporate, etc.) and location of the main office

Other changes ※The submission destination is different.

(1) Matters related to claims for benefits, etc. (Change in the name of the transfer account, etc.)
