In case of emergency, medical institutions other than the designated medical institution can also be consulted.

This is a system that pays public funds. It applies only when you are admitted to a designated child care institution.
We can confirm designated child care institution in Yokohama city in the following list.
If you are hospitalized at a medical institution outside Yokohama, please check with the medical institution or the local government where the medical institution is located.

Premature babies with Address in Yokohama (Note 1) fall under any of the following 1 or 2 and must be admitted to a designated child care institution to receive child care.

From the day when you are hospitalized at a designated nursing care institution and start raising premature babies until you leave the hospital (up to two days before your first birthday)
Within the above range, the period will be determined based on the scheduled medical treatment period described by the doctor in the “Childcare and Medical Opinion Statement” submitted at the time of application.

However, if you leave the hospital before the expiration of the period, it will end on the day of discharge.
(If you are transferred to another designated child care institution for medical unavoidable reasons, you will be treated continuously by applying again. To apply for a transfer.
A new child care statement from a doctor at the transfer destination medical institution is required. )

Hospitalization medical expenses at a designated nursing care institution (co-payment for insurance medical treatment and co-payment for dietary and medical expenses at the time of hospitalization) is eligible for payment.
(examination, medicine, therapeutic materials, medical treatment, surgery, and other treatments)

Costs not covered by employee health insurance, such as diaper charges, differential bed charges, and care products prepared by medical institutions, are not covered.
(If you are not sure if you are covered by insurance, check with your medical institution or employee health insurance.)
In addition, items that have already been paid by medical institutions are not eligible. Refund is not possible.

When child care is approved, the parent's burden for child care is determined according to the parents' income, but in Yokohama City, all 0-year-old children are 0 years old.
Regardless of the parent's income, it is subject to the Subsidies for Medical Expenses for Pediatric Treatment System. If a child care ticket is presented, this parent's share will be paid.
Yokohama City pays directly as Subsidies for Medical Expenses for Pediatric Treatment, and there is no payment for parents at medical institutions.

However, if the child care ticket is not presented at the medical institution window or if the application is rejected, it will not be treated as child care.
Payment will be made as a normal insurance medical treatment as follows. (Excluding social security.)
（１） When hospitalized at a medical institution in Kanagawa Prefecture
　　As a general medical treatment, the payment will be made by presenting the Medical Certificate for infant "Yokohama Maru Milk Medical Certificate" in Yokohama along with the health insurance card (conventional health insurance card, minor insurance card or qualification confirmation).
　　Medical Certificate for infant must apply to the ward office Insurance and Pension Division of your ward and receive it.
（２） When hospitalized at a medical institution outside the prefecture
　　Medical Certificate for infant in Yokohama cannot be used outside the prefecture. After payment, you will need to apply for a refund at the ward office Insurance and Pension Division.
　　※The cost of inpatient treatment at NICU may be high, but there is a limit to the amount of insurance medical treatment at the counter according to your income.
　　medical expenses, which has been paid in excess of the limit at the counter, will be paid at a later date upon application to employee health insurance.
　 If you apply to employee health insurance in advance and receive a limit application certificate, you can limit the amount paid at the counter to the limit.

　　※If you have changed your health insurance card between the birth of your child and the time you apply for childcare, you can check the contents of the new employee health insurance (conventional health insurance card, qualification confirmation, notification of qualification information, Please bring a copy of the insurance card information screen of the minor portal, etc.) with the insurer number, symbol number, etc. before the change.

６．My number (personal number) confirmation document of child and protector
　　In the case of a My Number Notification Card, a public certificate with a photo of your face (driver's license, passport, etc.) is also required.
　　Please present it at the counter to confirm the identity of the applicant and the items on the application form.

４．If you leave the hospital or continue to be hospitalized at the age of one year (the day before your birthday), your child medical care benefits will end.

５．If your Address or insurance card changes, or if you lose your childcare ticket, please apply for a reissue at the ward office Children and Families Support Division.
