medical expenses subsidies for single-parent families

This is a system in which Yokohama City pays a portion of the co-payment when a single-parent family, such as a mother-child family or a father-son family who joins the employee health insurance receives a doctor due to illness or injury.
Welfare Medical Certificate will be given to those who are eligible.

It is not applicable in the following cases.

• If you are receiving welfare
• If you are in a child welfare facility (including foster parents and foster children)

By submitting the welfare Medical Certificate and insurance card to windows such as hospitals, you can receive medical treatment without partial contribution.

In order to receive the medical expenses subsidy for single-parent families, please bring the following and apply to the ward office Insurance and Pension Division National Health Insurance Section.

• Child Raising Allowance Certificates
• Health insurance card

If you do not have a Child Raising Allowance certificate, please attach the following documents:

• certificate of family register
• Taxable (income) certificate for two years before (for all matters)

• Without a health insurance card, welfare Medical Certificate cannot be used.
• Expenses that are not covered by insurance medical treatment, such as extra beds during hospitalization, and out-of-pocket expenses (standard burden) for meals at the time of hospitalization are not subsidized.
• Please note that five years after consultation, the statute of limitations will expire and you will not be able to apply.
• If employee health insurance receives high medical costs or additional benefits, the amount will be paid excluding that amount.

・When it is no longer a single-parent family
・Address, when his name changes
・When you enter (or leave) a child welfare facility
・When receiving another medical expenses subsidy program (medical expenses Severely Disabled Persons)
