For those affected by the disaster

You may lose your health insurance card or nursing ticket or evacuate while leaving it in your house, so you can consult a doctor by claiming your name, date of birth, Address, etc.

In case of emergency, you can consult a medical institution other than the designated medical institution.

What is the elementary school student Rehabilitation medical care benefits System?

 For those who have osteoarthritis tuberculosis and other tuberculosis and need hospitalization, the city pays the medical expenses necessary for the treatment and provides learning supplies and daily necessities.
 In addition, depending on the income tax amount of the household, there is a co-payment.
 However, you will not be able to receive a system that is not a designated nursing care medical institution.
※Please contact the medical institution or the Health and Social Welfare Bureau Medical Aid Division to determine if the hospital where your child is hospitalized falls under a designated nursing care institution.

 There is no need to pay at the hospital counter as the city will bear the treatment of tuberculosis.
 However, treatments other than tuberculosis treatment and non-insurance treatments such as differential beds, are not covered by medical care benefits and must be paid at the counter.
 In addition, co-payment occurs according to income tax amount of household, but this will have you pay based on request from city at a later date.
 In addition, we will provide daily necessities necessary for recuperation life and school supplies necessary for receiving school education.

Application Flow

 Please contact the Health and Social Welfare Bureau Medical Aid Division or Health and Welfare Center Children and Families Support Division in your ward in advance.

 At the Health and Welfare Center Children and Families Support Division window where you live, you will receive an "Application Form" and a "Nursing Statement" such as "required documents". Please prepare the documents and submit it to the counter.
(Note: Please submit it to the Health and Welfare Center of your ward.)

 If you bring the following items, you can fill out the other required documents at the counter.
◆A written opinion written by family doctor
◆A copy of your child's health insurance card
◆Proof of income (taxation certificate, withholding slip or tax return)
(Documents are required for all incomes in same household.)
(Those who are taxed in Yokohama City can also be taken at the ward office.)
◆Your child's personal number card (My Number Card) or Personal Number Notification Card
(Please bring something that can identify you.)
◆Parent's personal number card (my number card) or personal number notification card
(Please bring something that can identify you. Example: driver's license, etc.)

 As a result of examination, we mail "Yokohama-shi nursing medical ticket" to authorized one in three to four weeks from application date.
Please be sure to show it at the medical institution window as soon as you receive the "Yokohama City Nursing Care Medical Ticket" arrives.
