I would like to express my heartfelt sympathy to all those affected by the Noto Peninsula Earthquake that occurred on January 1, 2024.
Since the Disaster Relief Law has been applied in some areas of Niigata, Toyama, Ishikawa and Fukui prefectures, the handling of publicly funded medical care (care for nursing care) when affected people are evacuated to Motoichi Will be announced as follows.

You may be able to have a medical examination by claiming your name, date of birth, Address, etc. because your health insurance card or nursing ticket may be lost or evacuated while leaving it in your house.

In case of emergency, medical institutions other than the designated medical institution can also be consulted.

What is the medical care benefits system for tuberculosis elementary school student nursing care?

 For those who have bone joint tuberculosis and other tuberculosis and need hospitalization, the city pays the medical expenses necessary for the treatment and provides learning supplies and daily necessities.
 In addition, co-payments will be incurred depending on the household's income tax amount.
 However, you cannot receive this system if you are not a designated nursing medical institution.

・Be under 18 years of age
・The parent of elementary school student has a Address in Yokohama.
・Having osteoarthritis and other tuberculosis, which requires a particularly long time to treat it.
・The doctor admits that hospitalization is necessary

 There is no need to pay at the hospital counter as the city will bear the treatment of tuberculosis covered by insurance.
 However, treatments other than tuberculosis treatment and non-insurance treatment such as differential bed charges are not covered by medical care benefits and must be paid at the counter.
 In addition, co-payment occurs according to income tax amount of household, but this will have you pay based on request from city later.
 In addition, we will provide the target people with daily necessities necessary for recuperation life and school supplies necessary for receiving school education.

Flow of Application

 Please contact Health and Social Welfare Bureau Medical Aid Division or Health and Welfare Center Children and Families Support Division in your ward in advance.

 At the Children and Families Support Division office in Health and Welfare Center, where you live, you will be given a required documents application form and a written opinion for nursing care. Please prepare the documents and submit them to the counter.
(Note: Please submit to Health and Welfare Center of your ward.)

 If you bring the following, you can fill in the other required documents at the counter.
◆“Nursing Opinion Statement” written by family doctor
◆A copy of your child's health insurance card
◆Income certificate (copy of taxation certificate, withholding slip or tax return)
(You need documents from all those who have income in same household.)
(If you are taxed in Yokohama City, you can also take it at the ward office.)
◆Your child's personal number card (My Number Card) or personal number notification card
(Please bring something that can identify you.)
◆Parents' personal number card (My Number Card) or personal number notification card
(Please bring something that can identify you for example: driver's license, etc.)

 As a result of examination, we mail "Yokohama-shi nursing care medical ticket" to authorized one within 3-4 weeks from application day.
As soon as you receive the "Yokohama City Nursing Care Ticket", be sure to show it at the medical institution window.
