About reservation
In this hospital, in order to shorten the waiting time for patients, reservations are required in principle.
If you wish to have a medical treatment, please make an appointment in advance.
Anyone can make a reservation, but you need basic information such as the patient's name, date of birth, Address and Phone number.
In addition, medical treatment may not be possible at the appointment time due to outpatient consultation status, emergency patients' response, emergency surgery, and sudden changes in in hospitalized patients.

※Brain disease (headache, dizziness, convulsions, etc.) and spinal cord disease (low back pain, etc.) can be consulted without an appointment, but a doctor cannot be specified.
※In addition, reservations will be given priority, so if you are not booked, you may have to wait.
Non-insurance combined medical expenses (selected medical treatment)
If there is no reservation or continuous outpatient clinic for 3 months and you go to the hospital again, in principle, it will be treated the same as first visit (Shoshin), and if you do not have a letter of introduction from another medical institution, you will be required to pay 7,700 yen (tax included).
In addition, after giving a letter of introduction from our hospital to another medical institution, if you visit our hospital without a letter of introduction from another medical institution, you will be required to pay 3,300 yen (tax included).
Flow of the day
Confirmation of insurance card
Please bring patient's card Shinryo-ken, my number card insurance card (or health insurance card) and various Medical Certificate (incurable disease Medical Certificate, independence support Medical Certificate, etc.).
You will need to check your insurance each time you visit. When the terminal operation of the minor insurance card is completed, be sure to call the reception.
If there is a change in Address or Phone number, please let us know at the insurance card confirmation window.
If you do not bring your patient's card Shinryo-ken on the day, it will be reissued and a fee of 220 yen will be charged.
If you have reserved only blood collection, X-ray, or electrocardiogram, please check your insurance card and go directly to the laboratory.
Return reception
If you have a reservation in second or subsequent visit (Saishin), please pass the patient's card Shinryo-ken through the return machine.
The return machine is located near the front entrance on the first floor and in front of the rehabilitation reception on the first basement floor.
Outpatient reception
Patients with reservations should present the reception slip output from the return machine to the outpatient reception desk and accept it.
If you do not have a reservation, please tell the outpatient reception that you have come to the non-reservation consultation, present patient's card Shinryo-ken, and accept it.
After completing the reception, you will receive the basic outpatient form and patient's card Shinryo-ken, and wait for the order of the consultation in front of the consultation room.
When your turn comes, enter the examination room and see a doctor.
The outpatient reception desk is located on the first floor and the rehabilitation outpatient reception desk is located on the first basement floor.
Accounting
When all consultations and examinations are completed, submit a basic outpatient slip to the accounting reception and receive a receipt number slip.
If the number of the receipt number slip is displayed on the accounting information display board, please pay with the automatic payment machine.
You can choose to pay by cash and credit card.
