About reduction of user burden by social welfare corporation

Reducing the burden on users by social welfare corporations is a system in which users who use The Long-term Care Insurance services operated by social corporations can receive measures to reduce user contributions if they meet certain income standards.

Requirements for Grants

Grantees

(1) Municipal tax-exempt households who fall under all of the following a to o
 Oh, income standard
  Single-person households must be less than 1.5 million yen per year (500,000 yen added for each additional household member).
 B. Asset Standards
  Single-person households must be less than 3.5 million yen (the sum of 1 million yen for each additional household member).
 Do not own real estate other than residential land (less than 200m2) and houses.
 D) Not being supported by relatives who have the ability to bear the burden (family and relatives have not received dependent deductions for the person).
 Long-term Care Insurance Premium must not be in arrears in the previous year of the year to which the application date belongs.
(2) Recipients of social security / Boundary Layer

Subject services

The following 16 services provided by social welfare corporations that offer reduction implementation
(1) Nursing homes for the elderly [nursing homes for the elderly]
(2) Day care [day service (elderly day care)]
(3) Short-term entrance life care [short stay] (including services pertaining to the Yokohama City Life Support Short Stay Business Implementation Guidelines)
(4) Visiting care (including services pertaining to the Yokohama City Home Life Support Home Help Project Implementation Guidelines)
(5) Care prevention short-term entrance life care
(6) Preventive care Dementia responsive outpatient care
(7) Prevention of long-term care Small-scale multifunctional home care
(8) Night-time home-visit care
(9) Dementia-responsive outpatient care
(10) Small-scale multifunctional home care
(11) Community-based nursing homes for the elderly
(12) Regular patrol / ad-hoc home-visit nursing care
(13) Nursing small-scale multifunctional home care
(14) Community-based day care
(15) Among the first visit business, the business equivalent to the old care preventive visit nursing care (the self-pay ratio is limited to the same as insurance benefits.)
(16) Out of the first tsusho business, the business equivalent to the old care prevention tsusho care (the self-pay ratio is limited to the same as insurance benefits.)

Expenses to be subsidized

Reduce the user's burden ([A] 10% of the cost required for the target service specified by the provisions of the Nursing Care Insurance Law [B] food expenses [C] living expenses) at the reduction rate indicated on the confirmation certificate .
※However, for social security recipients, only living expenses for unit-type private rooms and conventional private rooms are eligible for reduction.

Grant period

From the first day of the month to which the application date belongs to July 31 of the year following the year to which the application date belongs
Until (If the month to which the application date belongs is between April and July, July 31 of the relevant year).
The grant period is up to one year, and expires at the end of July every year. If you want to continue receiving the grant after the end of the grant period, you need to renew.
Renewal application period: From June 1 to August 31
※For those who have the relevant confirmation certificate as of April every year, we will send you a notice of annual renewal from Yokohama City by mid-June.
 If you have sent your application after April, you will need to apply for a confirmation certificate from August 1 again.

Please download and fill out the application form below, and submit it to the Health and Social Welfare Bureau Senior Facilities Division by mail.
※Please note that the documents to be submitted vary depending on the category.

 2** (From August 1, 2025 to July 31, 2026) ※The application acceptance period will be from June 2025.

 ・Application for reduction of user burden by social welfare corporation (third style)
 1⃣(From August 1, 2024 to July 31, 2025 (PDF: 217KB)
 2** (From August 1, 2025 to July 31, 2026) ※The application acceptance period will be from June 2025.
 Income and Asset Status Report and Consent Form (No. 4 style) (PDF: 232KB)
 ・Copy of documents showing the estimated amount of income for one year in the year to which the application date belongs (such as pension transfer notice and business income proof documents / for all households)
 ・Copy of documents that show assets (the latest balance page of bankbook, etc. / for all households)
