Among those who are insured by The Long-term Care Insurance in Yokohama City who are certified as requiring long-term care, those who meet all of the following requirements are eligible for the grant.

1. Equivalent to the 5th to 7th stages of the Long-term Care Insurance Premium stage in Yokohama City (those who are exempt from municipal tax and have a municipal tax taxpayer in the same household, or the person is a municipal tax taxpayer and the person's premium calculated income is less than 1.2 million yen).
2. Those who have not received the burden limit certification and those who have not received special reduction measures for taxable persons.
3. Those who have a total asset of 5 million yen or less (if there is a spouse, the total asset of the couple is 15 million yen or less). In the case of Long-term Care Insurance Premium No. 2 insured persons, the total amount of assets is 10 million yen or less (if there is a spouse, the total amount of assets of the couple is 20 million yen or less).
4. The grantee and spouse do not own real estate other than those specified below.

5. Don't be behind with Long-term Care Insurance Premium.

This subsidy covers living expenses when using unit-type private rooms (excluding unit-type private rooms multi-bed rooms) in unit-type special elderly nursing homes in Yokohama City.
The amount eligible for the grant is the difference between the living expenses (★) set by the facility and the unit-type private room in the 3rd stage 2 of the The Long-term Care Insurance Burden Limit Certification.
★If the amount exceeds the standard cost of unit-type private room living expenses set by the government, the amount that can be subsidized is the difference between the standard cost of unit-type private room living expenses set by the government and the burden limit of unit-type private room in step 3 of the The Long-term Care Insurance Burden Limit Certification.

Example I When the living expenses specified by the facility are 2,000 yen (residential expenses specified by the facility <national standard expenses amount)
　　　2,000 yen (residential expenses determined by the facility)-1,370 yen (the maximum burden of third stage 2) = 630 yen
Example II When the living expenses specified by the facility are 3,000 yen (the living expenses specified by the facility)＞National base cost
　　　2,066 yen (national standard cost)-1,370 yen (3rd stage 2 burden limit) = 696 yen

From the first day of the month to which the application date belongs to July 31 of the year following the year to which the application date belongs (if the month to which the application date belongs is between April and July, July 31 of the relevant year).
The grant period is up to one year, and expires at the end of July every year. If you want to continue receiving the grant after the end of the grant period, you need to renew.
For those who already have the relevant letter of advice, we will send you a year update from Yokohama City by mid-June.
※If you apply from July 1, 2024 to September 30, 2024 (must arrive), August 1, 2024 will be the grant application start date.
※If you apply after October 1, 2024 and are eligible for the grant, the first day of the month to which the application date belongs will be the grant start date.

Once you have filled out the application form, we will send you another required documents by mail.

We send "confirmation letter of advice targeted for unit type special elderly nursing home facility residence costs furtherance" from Yokohama-shi.
Please confirm approval / non-approval in the notice.

Those who have received the notice stating "Approved" can receive the subsidy by presenting the notice to the facility.
(We receive monthly billing with the amount obtained by subtracting the grant amount from the facility.)

• A copy of documents (the latest balance page of the bankbook, etc.) that show the assets of the person and his / her spouse who are going to receive the grant, trusts, securities and other assets (such as the latest balance page of the bankbook)

■Submit only if the recipient of the grant is The Long-term Care Insurance No. 2 insured person

• A copy of documents (bankbooks, business income proof documents, etc.) that show the total income amount during the previous year of the year to which the application date belongs

※In principle, applications are accepted by mail. Please note that inquiries and applications are not accepted at the ward office window of each ward.
