23-valent pneumococcal vaccination subsidy project

Business

In Yokohama-shi, we subsidize a part of inoculation cost of 23-valent pneumococcal vaccine for the purpose of preventing seriousness when we suffer from pneumonia for person having notebook of internal dysfunction.
"Pneumococcal vaccine Vaccinations for adults" was started as "regular vaccination" for elderly people and those with specific disabilities by the Immunization Law enforcement order revision from October 1, 2014. Along with that, the system was reviewed from October 1, 2014, and the target persons were changed as follows.

※During 2024, coupons will be sent out sequentially to those who have newly received a certificate of the physically disabled for internal dysfunction and who meet the conditions of the target person.
※The following information and coupons are sent to the target person together.

About 23-valent pneumococcal vaccination expense subsidy program


Target person

Those who fall under the following age as of October 1 and who have Address in Yokohama City at the time of vaccination and fall under the following (1) or (2)
(1)Those who are 5 years old or older and under 60 years old and have a certificate of the physically disabled and fall under any of the functions of the heart, kidney, respiratory organs, bladder or rectum, small intestine, immunity by human immunodeficiency virus or liver Who
(2)Those aged between 60 and under 65 who have a certificate of the physically disabled.
(a) Those who fall under any of the functions of bladder or rectum, small intestine or liver
(b) Those who fall under any of the functions of heart, kidney, respiratory organs, or immunity caused by the human immunodeficiency virus, those who fall under grades 2 to 4
※However, please note that the following cases are not eligible for this system.
● Those who have moved out of Yokohama at the time of receiving vaccination
● Those who do not meet any of the above conditions (1) and (2) at the time of vaccination
● Those who have not renewed the vaccination beyond the deadline for re-judgment of the certificate of the physically disabled at the time of vaccination
● Patients with spleenectomy (for insurance benefits)
● Pollution-certified patients who have been issued the Pollution Medical Handbook (for pollution medical fees)

Subsidy expenses

The total vaccination cost (which costs more than 3,000 yen) depends on each Cooperating Medical Institutions, so please check in advance by phone or other means.
Costs exceeding 3,000 yen will be borne by you (there is no exemption from copayment) 。

If you wish to reissue a coupon due to loss, damage, soiling, expiration date, or wish to re-vaccinate at intervals of 5 years or more, please apply to the Health and Social Welfare Bureau Disability Independence Support Section with a copy of the certificate.

[When you receive a vaccination at a medical institution outside Yokohama / When you receive a vaccination at Cooperating Medical Institutions in the city without using a coupon]
If you do not use a coupon when you receive a vaccination at a medical institution outside the city or when you receive a vaccination at Cooperating Medical Institutions in the city. ※Only, you can request a request for the amount of the grant directly to Yokohama City (this is called "exception of the grant") 。
