Issuance of welfare taxi tickets

• Lending, transfer, or copying of this ticket is prohibited.
• If misconduct is found, we will collect this ticket and request a refund of the grant amount, or will not issue it for one year thereafter.
• This ticket cannot be redeemed.

We will issue a welfare taxi ticket that subsidizes up to 500 yen per ticket. Up to 7 welfare taxi tickets can be used for each ride.
In addition, even if you use a welfare taxi ticket, you can receive a "taxi fare discount". In addition, the use is limited to the granter (it can be used even if there is a passenger).

Those who live in the city corresponding to any of the following and have not received a special welfare ticket, a special elderly ticket, a special ticket, or a car fuel ticket for the disabled
（１） Those who have a 1.2 grade certificate of the physically disabled, including lower limbs, trunk, sight, internal, or mobility dysfunction
（２） Love Notebook (Nursing Handbook) Those who have A1 or A2 or those who have been determined to have an intelligence index of 35 or less at the Rehabilitation Consultation Center for Persons with Disabilities, children's guidance office
（３） Those who have a grade 3 certificate of the physically disabled, including lower limbs, trunk, sight, internal, or mobility dysfunction, who have a love notebook (nursing notebook) B1 or who are judged to have an intelligence index of 50 or less at the Rehabilitation Consultation Center / children's guidance office
（４） Those who have the Mental Handicapped Health and Welfare Handbook Level 1

From October 1 to September 30 of the following year, 84 cards will be issued annually.
Among those who are eligible, those who use a taxi to go to dialysis at least three times a week for renal dysfunction will receive 168 tickets per year.

When boarding, please show the certificate of the physically handicapped, the notebook of love or the health and welfare notebook for the mentally handicapped, and submit a welfare taxi ticket with the notebook number to the crew.

Bring your notebook to Health and Welfare Center each ward.

The welfare taxi ticket can be used only by the granter (it can be used even if there is a passenger). Never lend or transfer it to others, including your family. (We may ask you to collect your ticket as invalid and then suspend grant or refund the amount of misused.)

The welfare taxi ticket for the next validity period will be mailed to eligible persons by mid-September, so there is no need to apply again.
※If you do not have a valid notebook after October 1, you will not be able to issue a welfare taxi ticket, so if you need to renew it, please update the notebook as soon as possible.

The remaining number of welfare taxi tickets is 7 (14 for those who receive 168 tickets due to renal dysfunction) = 1 month's worth and the start month of use of special welfare tickets etc. is determined, so the remaining number is insufficient It cannot be switched until the month when there is.
※Switching is limited to one time during the validity period.
※As welfare special ticket, respect for the elderly special ticket, disability car fuel ticket, etc. cannot be paid together, welfare taxi ticket must be returned.
※If you switch to a special welfare ticket, you will be charged a user fee of 1,200 yen per year (600 yen under 20 years old).
(There is no user contribution when switching to a special ride certificate for the elderly.)

(※) For those who receive 168 copies due to kidney dysfunction, the "remaining number" in the table will be doubled.
