The Long-term Care Insurance services available to those who need support 1 and support 2

Home service

1.Preventive care home visits (home help)

We provide bathing, excretion and meal assistance so that home helper (home-care worker) can visit their homes and carry out independent daily life at home. (Assistance for getting on and off, such as going to the hospital is not available.)

2.Preventive care visits Bathing care

Nursing care staff and nursing staff visit the home and provide bathtubs and care for bathing.

3.Preventive care home nursing care

Nurses and others visit their homes according to the instructions of family doctor.

4.Preventive care visit rehabilitation

Physiotherapists, occupational therapists, etc. will visit the home and perform necessary rehabilitation under the direction of family doctor.

5.Care prevention outpatient care (day service (elderly day care))

We go to day service (elderly day care) facilities and receive services such as bathing and meals and functional training.
※Fixed monthly fee (multiple offices cannot be used)

6.Care prevention outpatient rehabilitation (day care)

I go to nursing care facilities for the elderly and hospitals and receive rehabilitation.
※Fixed monthly fee (multiple offices cannot be used)

7.Lending (rental) care prevention equipment

We lend welfare equipment to help you become more independent in daily life, such as handrails and walking aids.
※Rental of wheelchairs, electric beds, etc. is not available in principle for those requiring support 1 and support 2, except in certain exceptions.

8.Care prevention short-term entrance life care (short stay of welfare facilities)

Enter a welfare facility for a short period of time and receive services such as nursing care and functional training in daily life.
※If you need to use a short stay urgently due to sudden illness of the caregiver, consult your care manager. If you do not have a care manager, please contact Elderly and Disabled Support Division, Izumi Ward Office.

9.Care prevention short-term admission care (short stays at medical facilities, etc.)

We stay at nursing homes and hospitals for a short period of time, and receive services such as nursing care and functional training under medical management.
※If you need to use a short stay urgently due to sudden illness of the caregiver, consult your care manager. If you do not have a care manager, please contact Elderly and Disabled Support Division, Izumi Ward Office.

10. Care prevention home care management guidance

Doctors, dentists, pharmacists, etc. will visit and provide medical management and guidance.

11. Nursing care prevention specific facilities Resident life care

We provide nursing care and guidance for daily life to those who are in paid nursing homes.

12. Purchase of special care preventive welfare equipment

If you purchase welfare equipment for excretion and bathing that is not compatible with lending from a designated company, the purchase cost will be refunded.
※Purchase limit: Since 100,000 yen per year (including consumption tax), the maximum refund limit is 90,000 yen. In addition, if you purchase the same type of equipment in the same year, you will not be eligible in principle.

13. Renovation of preventive care housing

Renovation costs will be refunded in the case of small-scale house renovation such as installing handrails and eliminating steps.
※When we perform house repair, report of repair contents is necessary beforehand.

Community-based services

It can be used only by Yokohama City insured persons. (If you have Address in other cities, you cannot use it in principle.)

1.Day care for preventive care for dementia

Dementia goes to the office and receives services such as nursing care and functional training such as bathing and meals.

2.Small-scale multi-functional home care

We receive services such as bathing and meals, and functional training, combining visits and lodgings at any time, mainly on going to offices.

3.Collaborative care for dementia (group home)

Dementia receives services such as daily life care and functional training while living together with 5 to 9 people in a homey atmosphere.
※Limited to those who need support 2.
