In other cases where procedures are required

If a person in The Long-term Care Insurance has moved, lost or died of a long-term care insurance card, the following procedures are required:

When you move into Izumi Ward from another municipality

Procedure location

Insurance and Pension Division National Health Insurance Section (Window 205 on the 2nd floor of Izumi Ward Office)

What is required for the procedure

  • Identity verification documents (driver's license, my number card, etc.) of those who can come to the counter
  • Certificate of eligibility (if issued by the previous municipality)
  • Power of Attorney (not required if the person coming to the counter is the insured person and the person living in the same household on resident certificate with the insured person)

※Please submit the resident registration notice (204 window on the 2nd floor of Registration Section Family Registry Division) first.

When you move into Izumi Ward from another ward in Yokohama city

Procedure location

Insurance and Pension Division National Health Insurance Section (Window 205 on the 2nd floor of Izumi Ward Office)

What is required for the procedure

  • Identity verification documents (driver's license, my number card, etc.) of those who can come to the counter
  • Long-term care insurance card (If you have any other items that have been issued such as The Long-term Care Insurance burden ratio certificate or The Long-term Care Insurance burden limit certificate, please bring them with you.)
  • Power of Attorney (not required if the person coming to the counter is the insured person and the person living in the same household on resident certificate with the insured person)

When I changed Address in Izumi Ward

Procedure location

※If you have been certified as requiring nursing care or support, we will issue a certificate of eligibility. Please submit it to the municipal office where you are moving to.

Loss of long-term care insurance card

Procedure location

What is required for the procedure

  • Identity verification documents (driver's license, my number card, etc.) of those who can come to the counter
  • Power of Attorney (not required if the person coming to the counter is the insured person and the person living in the same household on resident certificate with the insured person)
  • Documents with name, insured number, etc., such as notices about The Long-term Care Insurance (if any)

※If lost outside the house, be sure to notify the police as it may be abused.

When the subject of The Long-term Care Insurance died

What is required for the procedure

  • Identity verification documents (driver's license, my number card, etc.) of those who can come to the counter
  • Long-term care insurance card (If you have any other items that have been issued such as The Long-term Care Insurance burden ratio certificate or The Long-term Care Insurance burden limit certificate, please bring them with you.)
  • death certificate (unnecessary if you have already sent death registration/death notification form to the Izumi Ward Office)
