If a person in The Long-term Care Insurance has moved, lost or died of a long-term care insurance card, the following procedures are required:

When you move into Izumi Ward from another municipality

Procedure location

Insurance and Pension Division National Health Insurance Section (Window 205 on the 2nd floor of Izumi Ward Office)

What is required for the procedure

  • Certificate of eligibility (Only those who have already been certified as requiring nursing care and support in other municipalities will be issued by the previous municipalities)
  • private seal (Approved and permitted) (Not required if the applicant applies)

※Please submit the resident registration notice (204 window on the 2nd floor of Registration Section Family Registry Division) first.

When you move into Izumi Ward from another ward in Yokohama city

Procedure location

Insurance and Pension Division National Health Insurance Section (Window 205 on the 2nd floor of Izumi Ward Office)

What is required for the procedure

  • Long-term care insurance card
  • private seal (Approved and permitted) (Not required if the applicant applies)

When I changed Address in Izumi Ward

※If you have been certified as requiring nursing care or support, we will issue a certificate of eligibility. Please submit it to the municipal office where you are moving to.

Loss of long-term care insurance card

Procedure location

Insurance and Pension Division National Health Insurance Section (Window 205 on the 2nd floor of Izumi Ward Office)

What is required for the procedure

  • Documents with name, insured number, etc., such as notices about The Long-term Care Insurance
  • Items that can identify you (driver's license, health insurance card, etc.)
  • private seal (acceptable)
  • Power of Attorney (if the person who comes to the notification is not the family of the The Long-term Care Insurance user)

※If lost outside the house, be sure to notify the police as it may be abused.

When the subject of The Long-term Care Insurance died

  • death certificate (unnecessary if you have already sent death registration/death notification form to the Izumi Ward Office)
