When moving into Izumi Ward from another municipality
Procedure Place
Insurance and Pension Division National Health Insurance Section (205, 2nd floor, Izumi Ward Office)
What is required for the procedure
- Certificate of Eligibility (Only those who have already been certified as requiring nursing care and support in other municipalities will be issued by the previous municipalities)
- private seal (permitted) (Not required if the applicant applies)
※Please submit a resident registration notice (204 window on the 2nd floor of Registration Section, Family Registry Division).
When moving into Izumi Ward from another ward in Yokohama City

※If you have been certified as requiring nursing care or support, we will issue a certificate of eligibility. Please submit it to the municipalities to which you are moving to.
When a long-term care insurance card is lost

What is required for the procedure
- Documents stating the name and insured number, etc. regarding The Long-term Care Insurance
- Identification of identity (driver's license, health insurance card, etc.)
- private seal (permitted)
- Power of Attorney (if the person coming to the notification is not the family of the The Long-term Care Insurance user)
※If lost outside the house, be sure to notify the police as it may be misused.
When the subject of The Long-term Care Insurance died
