Public health nurses at the Regional Comprehensive Support Center (Community Care Plaza) in charge of your area set the goals of the user, incorporate services for the prevention of long-term care, and create a care prevention care plan. The creation of a preventive care plan can be entrusted to a care manager of a designated home care support office based on the user's intention.

Those who have already used the home service
Please contact the Local Elderly Care Management Center / entrance facility requesting the creation of a care plan based on the certification result immediately.
