Public health nurses at the Regional Comprehensive Support Center (Community Care Plaza) in charge of the area where you live set goals for users, incorporate services for preventive care, and create a preventive care plan. The creation of a preventive care plan can also be entrusted to a care manager at a designated home care support office based on the user's intention.

Please consult with the Regional Comprehensive Support Center in charge of your district.
Those who are already using the home service
Please contact the Local Elderly Care Management Center, entrance facility, etc. requesting the creation of a care plan for the certification result immediately.
