About Hodogaya Ward's nursing care (support required) certification application window

In order to be in need of nursing care and to use The Long-term Care Insurance services, it is necessary to apply for nursing care and support certification at the ward office and community care plaza in your area.
For more information about the long-term care insurance system, please see "Overview of The Long-term Care Insurance".

Those who are eligible for The Long-term Care Insurance

  • Those who are 40 years old or older and under 65 years old and have a specific illness
  • 65 years old or older

What you need at the time of application

  • The Long-term Care Insurance insured person's card (medical insurance card for those aged 40 to under 65) ※If you have lost it, please let us know at the counter.
  • family doctor's name, medical institution name, location, and Phone number                               → Please bring your patient's card Shinryo-ken or memo so that you can see the above when filling out the application form.
  • Identification documents (when a person other than the person can come to the application)
  • Application for certification for nursing care and support required (in the case of reception at the counter, it is at the counter)

Application window

Applications are accepted at the ward office and the window of the community care plaza where you live. In addition, we can request designated home care support company, care insurance facility for agency of application presentation. It is also possible to apply by mail.

Hours: 8:45 am to 5:00 pm Monday to Friday (excluding holidays and New Year's holidays)

Withdrawal of application

If you are applying for certification under The Long-term Care Insurance, you can withdraw your application if you no longer need to use the The Long-term Care Insurance Service after applying for The Long-term Care Insurance, you can withdraw your application.
Please submit the withdrawal form to the person in charge of The Long-term Care Insurance, Hodogaya Ward Office.

About procedure after application for nursing care (support required) certification

About dispatch of certification letter of advice and care insurance card

After the examination committee, a notification of the certification result and a new The Long-term Care Insurance insured card will be sent to the registered place of Address by simple registered mail (no transfer required).
If it is difficult to receive it at the resident certificate registered location, please contact us below to change the destination.
