About Hodogaya Ward's nursing care (support required) certification application window

If you need nursing care and use The Long-term Care Insurance services, you will need to apply for certification for nursing care and support at the ward office and your local community care plaza.
Please refer to "Overview of The Long-term Care Insurance" for details of the long-term care insurance system.
Those who are eligible for The Long-term Care Insurance
- Those who are over 40 years old and under 65 years old and have a specific illness
- 65 years or older
Necessary items at the time of application
- The Long-term Care Insurance Insured Card (for those aged between 40 and under 65, medical insurance card) ※In case of loss, please let us know at the counter.
- Name, medical institution name, location, and Phone number of family doctor → Please bring patient's card Shinryo-ken and notes so that you can see the above when filling out the application form.
- Identity verification documents (if someone other than the person can come to the application)
- Application for certification for nursing care and support required (in the case of reception at the counter, there is at the counter)

※When filling out the form, please refer to the entry example and fill it out.
Application window
We accept application at ward office, window of community care plaza where we live. In addition, we can request designated home care support company, care insurance facility for agency of application presentation. You can also apply by mail.

Hours: 8:45 am to 5:00 pm Monday to Friday (excluding holidays and New Year holidays)

Weekdays and Saturdays: From 9 a.m. to 9 p.m.
Sunday: Holidays: From 9 a.m. to 5 p.m.

Withdrawal of Application
If you are applying for The Long-term Care Insurance certification, you can withdraw your application if you no longer need to use The Long-term Care Insurance Service after applying for The Long-term Care Insurance.
Please submit the withdrawal form to The Long-term Care Insurance, Hodogaya Ward Office.

About procedure after application for nursing care (support required) certification
About dispatch of authorization letter of advice and care insurance card
After the examination committee is completed, a notification of the certification result and a new The Long-term Care Insurance insured card will be sent to the registered location of Address by simple registered mail (no transfer required).
If it is difficult to receive it at the registered location of resident certificate, please contact us below to change the destination address.
