1 Preparation of documents to be submitted and various reports

Report form and spelling of necessary report (building, building equipment, fire prevention equipment)

  • When submitting a report, you will need a summary sheet and one copy of each report. A receipt will be issued at the time of reception. If you need a copy of the reporter, you can prepare one copy separately, and we will stamp the reception stamp at the reception and return it. If you would like to return your receipt and copy by mail, please prepare a reply envelope with a stamp attached at the time of submission.

※If you are a "private room video store, etc." is a target, please attach the "Survey Form (No. 4 style 2)".
※When "underground mall" is targeted, please attach "survey result vote (underground mall)" (Yokohama-shi notification 85th separate paragraph) in addition.

About various notifications

  • If a periodic report is no longer required due to a change in use or removal, or if it becomes necessary again due to a change in use, a notification is required when basic information such as the owner / manager is changed, or when the reporting time is changed.
