About the authorization schedule

1.Schedule for pre-examination

A. Application Form (draft) Submission deadline
  Application form (draft) submission From Tuesday, May 7, 2024 to 17:15 on Friday, May 17, 2024
B. Submission method
  The conventional submission method is "bring", but in order to prevent the spread of the new coronavirus infection, submission by "mail" is also acceptable.
 ※When submitting by bringing, application will be accepted only if you bring it within the business hours of the deadline (until 17:15).
 ※When submitting by mail, please submit a letter pack or other delivery record, such as a letter pack, by deadline.
 (After arrival, Yokohama City will contact you, so if you do not contact us around May 22, please contact us for confirmation just in case.

C Other precautions
 ※For the contents of the documents, check the medical corporation establishment approval application checklist (attached sheet) (PDF: 220KB) carefully.

2.Contents of preliminary examination, etc.

B. Details of pre-examination
  For the application form (draft) that has been accepted, we will conduct a form review and problem arrangement.
  This screening will be carried out in a way that each person in charge adjusts the schedule individually with the submitter, consult with each other, and sort out the problems.
  We would like to ask for your cooperation in coordinating the meeting day.

3.Schedule after the preliminary examination

※A considerable number of applications are expected this time as well, so as a result of preliminary examination of the application form (draft), if it is judged that significant correction is required.

※Details of the autumn application procedure will be posted approximately one month before the reception starts.
