In the case of receiving routine immunization at a medical institution outside the city for homecoming births, etc., the “Vaccination Implementation Request Form”, which clarifies that the The Mayor of Yokohama is responsible for the implementation based on the Immunization Act.

If you have a resident registration in Nishi Ward as of the date of vaccination and wish to receive regular vaccinations at a medical institution outside Yokohama for homecoming birth or inpatient treatment, you can apply online via the link below.

If you have difficulty applying online, or if you do not fall under the above reasons but wish to inoculate outside the city, please checkHealth Promotion Section (Tel: 045-320-8439, FAX: 045-324-3703).

• It will take about one to two weeks from application to shipping.
• Health Promotion Section may contact you regarding your application details.
• Vaccinations outside the city are paid entirely by yourself, but if you are vaccinated outside the city for homecoming birth or inpatient treatment, you can get a refund (reimbursement payment) for the vaccination fee. We send application of repayment payment to target person along with "vaccination conduct request book".
• Vaccinations that have not been issued a “Vaccination Request Form” or vaccinations other than the implementation method specified in the Vaccination Law (target age, vaccination interval, etc.) are not eligible for refunds.
