Procedures that can be done without visiting the ward office (National Health Insurance, The Long-term Care Insurance, Medical System for Elderly Elderly, etc.)

In order to thoroughly prevent the spread of COVID-19, we have reduced the number of contact points.
Congestion is expected, and in order to prevent infectious diseases, please apply by mail for the following procedures. Please contact us for other procedures.
Depending on the content, the procedure may take some time. Thank you for your cooperation so that you can apply with plenty of time.
◎Notes
・Fill out the documents with an unerasable writing instrument such as a ballpoint pen (pencils and eraseable ballpoint pens are not allowed).
・For private seal, please use private seal, which uses vermilion. You can't use the stamp.
・Please note that in the case of incomplete documents, we may return the complete set of documents and ask you to resubmit.
